Birinci basamak hekimlerinin astım konusundaki bilgi ve yaklaşımları

Özet
İlimizde 1. basamakta sağlık hizmeti veren hekimlerin, astımın tanı ve tedavisine yaklaşımlarını değerlendirmek amacıyla bu çalışma planlanmıştır. Ankete ortalama çalışma süreleri 64.88 ± 39.42 ay olan 66 hekim alındı. Günlük ortalama poliklinik sayısı 60.67 ± 27.20 olan hekimlerin %66.6'sı, ülkemizde astım görülme sıklığını %11.64 ± 13.57 olarak tahmin etmişti, %34.8'i astımlı hastayla karşılaşma sıklığını ayda l olarak belirtmiş, sadece %4.5'i hastalara eğitim ve bilgi için 20 dakikadan fazla zaman ayırabildiklerini, %71.2'si hastalarının hiç kontrole gelmediğini ifade etmişlerdir. Akut astım tedavisinde %48.5 ile en çok in-haler beta-2 agonist, kronik astım tedavisinde ise %22.7 ile en çok inhaler beta-2 agonist + oral teofilin + inhaler kortikosteroid kombinasyonunun tercih edildiği görülmüştür. Çalışmamızın sonunda hekimlerimizin hastalara yeterince zaman ayıramadıkları, düzenli olarak hastalarını takip edemedikleri, tedavide yüksek oranda belirsizliklerin olduğu belirlenmiş, akut ve kronik astım tedavisinde, astım tedavi rehberin-de belirtilen protokollere yeterli uygunluğun sağlanamadığı görülmüştür. Sonuç olarak, gerek tıp eğitimi sırasında gerekse mezuniyet sonrası, hekimlere astım ile ilgili daha yaygın eğitim programlarının uygulanması ve bu konuda geliştirilen kılavuzların mutlaka ulaştırılması gerektiğini düşünmekteyiz.
Abstract
This study has been planned for evaluating the approaches of the first step physicians on the diagnosis and the therapy of asthma in our city. The 66 physicians whose employment periods were 64.88 ± 39.42 months were taken in the questionnaire. The mean number of patients who had been physical examined per a day was 60.67 ± 27.20. The 66% of all physicians who we-re taken in the questionnaire, had estimated the frequency of asthma about 11.64 ± 13.57%. 34.8% of them had determined that facing with the asthmatic patient was per a month. Only 45% of them had said that they had separated ti-me more than 20 minutes for education of patients. 71.2% of them determined that their patients never had come for the controls. It has been seen that in acute asthma therapy (i-2 agonist is the most preferred drug with a 48.5% percen-tage, in chronic asthma therapy (1-2 agonist + oral theophylline + inhaled corticosteroid are the most preferred drugs with a 22.7% percentage. In our study it has been determined that the physicians don't follow up their patients regularly, they don't have sufficient knowledge about the therapy, they don't fit the quide of asthma therapy. In conclusion we think that even during the medical education or post graduate education the physicians must be educated by more common education programmes and they must be get the guides about this subject.
