Yirmibeş pulmoner emboli olgusunun değerlendirilmesi

Özet
Bu çalışmada, kliniğimizde Ocak 1997-Eylül 1999 tarihleri arasında izlenen 25 pulmoner emboli olgusundaki klinik deneyimimiz değerlendirildi. Olguların tümü erkekti. Yaş ortalaması 49.4 ± 14.9 (30-73) yıl olarak bulundu. Önceden geçirilmiş tromboemboli (%12) ve geçirilmiş operasyon (%12) en sık tespit edilen predispozan faktörlerdi. En sık gözlenen semptom nefes darlığı (%84), en sık görülen radyolojik bulgu ise diyafragma yükselmesiydi (%76). Inisiyal arteryal kan gazı bakılabilen 17 olguda %71 oranında hipoksi, %52.9 oranında hipokapni saptandı. Ondokuz olguda perfüzyon sintigrafisi yapıldı ve 7 (%36.8) olguda yüksek olasılıklı, 5 (%26.3) olguda ise düşük olasılıklı perfüzyon defekti saptandı. Derin ven trombozu (DVT) açısından incelenen 21 olgunun 15'inde (%71.4) DVT tespit edildi. Laboratuvar bulguları içinde %36 oranında transaminaz ve laktat dehidrogenaz (LDH) yüksekliği saptandı. Hastanede yatış süresi ortalama 22.3 ± 12.3 (1-46) gün olarak bulundu. Onbeş (%60) hastaya düşük molekül ağırlıklı, 10 (%40) hastaya ise anfraksiyone heparin ile tedavi başlandı, iki hasta ek-situs oldu ve 4 hasta taburcu edildikten sonra izlenemedi. Takip edilen 19 hastanın 3'ünde (%15.7) pulmoner emboli tekrarladı
Abstract
In this report our clinic experience was evaluated among 25 patients with pulmonary embolism (PE) who were followed up between January 1997-September 1999 in our clinic. All of the cases were male and mean age was 49.4 ± 14.9 (30-73) years. The most common predisposing factors were previous history of thoromboembolism (12%) and surgery (12%). The most common symptom was dyspnea (84%) and the most common radiographie abnormality was elevation of diaphragm (76%). Initial arterial blood gasses analysis could have performed in 17 patients, in 9 (53%) of them hypocapnia and in 12 (71%) hypoxemia was found. Perfusion scan could performed in 19 patients and high probability lung scan defect was established in 7 (36.8%) patients. Twenty-one patients were evaluated for deep venous thrombosis (DVT) in 15 (71.4%) of them DVT was proofed. Among the laboratory findings 36% transaminase and serum lactic dehydrogenase elevation was found. The mean hospitalisation ti-me was 22.3 ± 12.3 (1 -46) days. In 15 (60%) patients, low-molecular weight heparin and in 10 (40%) patients unfractionated heparin was used. Two patients had died and 4 patients couldn't have followed. Recurrences was noticed 3 of 19 patients that we followed up.
