Plevral efüzyonun ayırıcı tanısında "Vasküler endotelyal 'Growth' faktör"ün tanısal değeri

Özet
Bu prospektif çalışmada, plevral sıvının ayırıcı tanısında "vasküler endotelyal 'growth' faktör (VEGF)"ün kullanılabilirliğinin değerlendirilmesi amaçlandı. Değişik etyolojiye sahip 100 plevral sivili hastanın plevral sıvı ve serum VEGF düzeyleri ELISA yöntemi ile ölçüldü. Tüm olguların yaş ortalamaları 56.59 ± 15.47 yıl idi. Eksüdatif sivili hastaların ortalama plevral sıvı VEGF düzeyleri (513.38 ± 583.30 pg/mL), transüdatif sivili olanlara göre (49.17 ± 73.98 pg/mL) anlamlı olarak yüksek bulundu (p 0.05). Sonuç olarak, çalışmamızda VEGF'nin özellikle malign plevral sıvılar olmak üzere eksüda niteliğindeki sıvıların oluşmasında rolü olabileceği düşünüldü. Ayrıca VEGF'nin plevral efüzyonlarda tanıya katkısının değerlendirilmesi için geniş kapsamlı çalışmaların yapılması gerektiği sonucuna varıldı.
Abstract
In this prospective study, our aim was to assess the usefullness of vascular endothelial growth factor (VEGF) in diagnosis of patients with pleural effusions. The levels of VEGF in pleural fluids from 100 patients with pleural effusions of varying origins were examined. The VEGF levels of pleural fluids of 100 patients with mean age of 56.59 ± 15.47 years were measured with an enzyme-linked immunosorbent assay. The median level of VEGF in the exudative effusions (513.38 ± 583.30 pg/mL) was significantly higher than the median level of VEGF in the transudative effusions (49.17 ± 73.98 pg/mL, p< 0.05). Malignant pleural effusions (885.50 ± 616.70 pg/mL) contained significantly higher levels of VEGF than benign pleural effusions (228.86 ± 409.85 pg/mL, p< 0.05). The median level of VEGF in the malignant effusions was also significantly higher than the median level of VEGF in the paramalignant effusions. There were no significant differences between pleural fluid VEGF levels of different histological subtypes of lung cancer (p> 0.05). In conclusion, it was thought that VEGF might play an important role in the formation of exudative effusions especially with malignant origin. We need further controlled studies to evaluate the value of VEGF in the diagnosis of pleural effusion.
