Tüberkülozlu 64 hasta ile temaslı kontrollerinden bulunan 97 yeni olgunun değerlendirilmesi

Özet
Tüberküloz (TB)'lu hasta ile temas hastalığın bulaşmasında bir risk faktörü olduğu için, temaslı kontrollerinin yapılması hem hastalıktan korunmada hem de yeni olguların saptanmasında önemlidir. Bu çalışma, dispanserimizde Ocak 1996-Aralık 2001 tarihleri arasında TB tanısı alan hastalardan 64 (grup 1)'ü ile onların temaslı kontrollerinden bulunan 97 (grup 2) yeni hastayı klinik ve sosyodemografik özellikleriyle değerlendirmek üzere planlandı. Yaş ortalamaları grup Tde 33.1 ± 2 yıl, grup 2'de 23.4 ± 15 yıl idi (t= 6.13, p< 0.05; SD= 159). Akciğer tutulumu grup 1'de 59 (%92.2), grup 2'de ise 63 (%64.9) hastada vardı (t= 1.58, p< 0.05; SD= 159). Yayma ve/veya kültür pozitifliği grup 1 'de 53 (%82.8) hastada, grup 2'de ise bakteriyolojik tetkik yapılan 75 hastanın 54 (%72)'ünde saptandı (t= 1.55, p< 0.05; SD= 137). Tanı süresi kayıtlı 49 grup 1 hastasının 9 (%18.4)'unda bir aydan kısa sürede tanı konmuştu. Grup 2'de ilk kontrolde (0-3 ay) tanı konan 38 (%39.2) hasta vardı. Grup 1 hastalarında en sık görülen bulgular öksürük, balgam ve gece terlemesi idi (%34.3). Bu gruptaki hastaların 403 temaslısından 359 (%89.1)'u kontrol edilmiş, tüberkülin deri testi 213 temaslının 161 (%75.6)'inde pozitif bulunmuş, 206 (%57.4)'sına kemoprofilaksi uygulanmıştı. Grup 2'de 21 (%21.6) hastanın BCG skarı yoktu. Hastaların 23 (%23.7)'üne kemoprofilaksi verilmiş, bunların 9 (%39.1)'u ilacı düzensiz kullanmıştı. Aile dışı temas öyküsü ile kemoprofilaksi uygulanmayanlar arasındaki ilişki anlamlı bulundu (p= 0.022). Bu gruptaki sigara içmeyen hastalarda kavitasyon daha az görüldü (%44'e karşı %20; p= 0.034). Grup Tde 50 (%80.6), grup 2'de 82 (%92.1) hasta tedavi düzenini aksatmayacak şekilde uyumlu idi (t= 1.64, p< 0.05; SD= 149). Çalışmamızın sonuçları, temaslı kontrolünün yeni olgu bulmada temel yöntemlerden biri olduğunu desteklerken, bu kontrolleri özellikle çocuklarda ve gençlerde sadece aile içi temaslılar ile sınırlamamak gerektiğini ve kemoprofilaksinin düzenli kullanılmasının önemini vurgulamaktadır.
Abstract
Since to contact with a tuberculosis (TB) patient is a risk factor for the transmission of TB, examination of the contacts of tuberculosis patients is important not only in the prevention of TB, but also for the diagnosis of new patients. This study is designed to evaluate clinical, social and demographic properties of 64 of the enrolled patients between January 1996-December 2001 .(group 1) and 97 patients, who were diagnosed during the examination of their contacts (group 2). The mean ages were 33.1 ± 2 in group 1, 23.4 ± 15 in group 2 (t= 6.13, p< 0.05; DF= 159). Fifty-nine (92.2%) patients in group 1 and 63 (64.9%) in group 2 had pulmonary TB (t= 1.58, p< 0.05; DF= 159). Sputum smear and/or culture of 53 (82.8%) patients were positive in group 1 and 54 (72%) of 75 patients, whose bacteriological monitoring were possible, in group 2 (t= 1.55, p< 0.05; DF= 137). Nine (18.4%) of 49 group 1 patients, whose symptomatic periods have been recorded, were diagnosed less than a period of a month. in group 2, 38 (39.2%) patients were diagnosed at the first clinic visit (between 0-3 months). The most frequent symptoms of group 1 patients were coughing, night sweating and expectorating sputum (34.3%). in the same group, 359 (89.1%) of 403 contacts were examined; 161 (75.6%) of 213 had positive tuberculin skin test, 206 (57.4%) were given chemoprophylaxis. in group 2, there was no bacillus Calmet-te-Guerin (BCC) vaccine scatrix in 21 (21.6%) patients. Twenty-three (23.7%) patients have been given cheomoprophylaxis, but 9 (39.1%) of them used the drugs irregularly. in this group, a significant relation was found between being a sporadic contact and not to be given chemoprophylaxis (p= 0.022). Also in this group, cavitation on chest radiograms of nonsmokers were less than smokers (44% versus 20%; p= 0.034). Fifty (80.6%) patients in group 1 and 82 (92.1%) in group 2 complied with the treatment (t= 1.64, p< 0.05; DF= 149). The results of the study confirmed that, contact examination is a basic method of finding new TB patients and emphasized the importance of examining not only the household, but also the sporadic contacts especially in childhood and teenage.
