Malign plevral efüzyonlarda tüp torakostomi ile talk plöredez uygulaması

Özet
Kemoterapiye dirençli tümörlerden kaynaklanan malign plevral efüzyonlar, ileri evre malign hastalıkların en önemli sorunlarından biridir. Başlangıçta yineleyen torasentezlerle izlenen olguların birtakım plöredez yöntemleri ile palyasyonu gelenek haline gelmiştir. Çoğunlukla standart palyasyon yöntemi olarak tüp torakostomi yolu ile kimyasal plöredez kullanılmaktadır. Bunun dışında video yardımlı torakoskopik cerrahi ile talk püskürtülmesi, kalıcı plevral kateter uygulanması, plörektomi, dekortikasyon ve abrazyon da kullanılmaktadır. Literatürde plöredezin gelişimi aşamasında çeşitli ajanların kulla-nıldığı görülmektedir. Bunların arasında bleomisin, tetrasiklin ve son olarak da talk en çok tercih edilen ajanlardır. Kliniğimizde 2002-2003 yıllan arasında malign plevral efüzyon tanısı almış 40 olguya talk plöredez uygulanmış ve palyasyon oranları belirlenmiştir, işlem öncesi intratorasik sıvıdan glikoz ve laktat dehidrogenaz (LDH) örnekleri alınmış, plöredez başarısına bu parametrelerin etkinliği değerlendirilmiştir. Olguların 26'sı erkek 14'ü kadın olup hastalar 28-80 yaş aralığında gruplanmıştı. En sık rastlanan komplikasyon ağrı, ateş ve sıkıntı hissi idi. PA akciğer grafisinde sinüs küntlüğü olguların 12'sinde izlenmiştir. Palyasyon başarısı 30-60-90 günlük periyodlara nüks takibi ile belirlenmiştir. Doksan günlük takip sonunda kümülatif olarak %92 oranında plöredez başarısı saptanmıştır. Bu oran diğer ajanlara göre talkın belirgin üstünlüğünü ortaya koymuştur
Abstract
Malignant pleural effusions originating from chemotherapy resistant tumors, are one of the major significant issues of the advanced malignant diseases. Repeating thoracentesis followed by the different pleurodesis technics became the traditional of a palliation. Chemical pleurodesis via tube thoracostomy is the standart method, which is used most commonly. Talc insufflation by means of video-assisted thoracic surgery, continuous intrapleural catheter application, pleurectomy, decortication, pleural abrasion are the other means of interventions. There have been many agents used, during the evolution of the .pleurodesis, all of which are cited on the literature. Bleomycin, tetracyclin and talc are the most commonly preferred agents.Forty patients, in whom talc pleurodesis were carried out in our department, between 2002-2003, were analyzed and the rate of palliation among the patients were documented. The glucose and the lactate dehydrogenase (LDH) levels de-tected in the pleural effusion before and after pleurodesis were ,used in order to success rate of talc pleuredosis.Twenty six male and 14 female patients, of whom ages between 28 and 80, constituted our research group. Chest pain, fever and feeling discomfort were the most common complications. Twelve patients had sinus obliteration on their chest X-rays. Palliation success rate were determined according to the presence of the recurrences in the 30th, 60th and the 90th days. After 90 days, the cumulative success rate of the talc pleurodesis was found as; 92%. Conclusively, this much higher success rate of the talc can be considered as its superiority to the other agents.
