Protein C, Protein S eksikliği, aktive protein C rezistansı ve faktör V leiden mutasyonuna bağlı tekrarlayan pulmoner tromboemboli olgusu

Özet
Otuz yaşında erkek hasta iki gündür olan öksürük, nefes darlığı, yan ağrısı ve 10 gün önce ortaya çıkan sol bacakta ağrı ve şişlik yakınmaları ile başvurdu. İki yıl önce de pulmoner tromboembolizm tanısıyla hastaneye yatırılmıştı. Bu nedenle hastada tekrarlayan pulmoner tromboembolizm düşünüldü. D-dimer yüksek bulundu. Toraks bilgisayarlı tomografi (BT) anjiyografisinde, sağ pulmoner arter distalinde lober ve segmental dallara uzanım gösteren, sol inen pulmoner arterde trombüs ile uyumlu hipodens alanlar görüldü. Ventilasyon/perfüzyon (V/P) sintigrafisinde sağ akciğerde ventilasyon defekti yokken multipl perfüzyon defektleri izlendi. Alt ekstremite Doppler ultrasonografide solda derin venlerde akut trombüs izlendi. Protein C ve protein S düzeyleri düşük bulundu. Antitrombin III normal düzeydeydi. Faktör V Leiden (FVL) mutasyonu (G1691A) ve ciddi aktive protein C rezistansı (APC-R) saptandı. Aile taramasında çocukları ile anne-babasında da protein C eksikliği, APC-R ve/veya FVL saptandı. Tekrarlayan pulmoner tromboemboli ve/veya venöz tromboz olgularında etyolojinin mutlaka araştırılması gerektiğini vurgulamak amacıyla sunulmuştur.
Abstract
Thirty-year old male patient admitted suffering from cough, chest pain, dyspnea for two days and swelling of his left leg for 10 days. He had hospitalization history of pulmonary thromboembolism two years ago. Accordingly, recurrent pulmonary embolism was considered. D-dimer concentration was high. There were thrombosis in both right pulmonary artery extended to lobar and segmental branches and left descendent pulmonary artery on his computed thorax angiography. On V/Q scintigraphy, multipl segmental perfusion defects with normal ventilation in right lung were observed. Multiple acute thrombosis in the deep venous system of the left lower extremity were established by Doppler ultrasound. Protein C and protein S levels were low. Antithrombin III was normal. Factor V Leiden (FVL) mutation (G1691A) was detected. There was severe activated protein C resistance (APC-R). Protein C deficiency, APC-R and FVL mutation were also found in his children and parents. The case was presented in order to emphasize that etiology should be investigated in recurrent pulmonary embolism and/or venous thrombosis cases.
