Özet

Allerjik bronkopulmoner aspergilioz (ABPA) duyarlı kişilerde bronşiyal sekresyonlarda Aspergillus sporlarının yerleşmesi sonrası oluşan bir hipersensitivite reaksiyonudur. Olguların büyük bir kısmında etken Aspergillus fumigatus'tur. Persistan astımlı veya kistik fibrozisli olgularda daha sık olarak görülür. Tanı klinik, radyolojik ve serolojik kriterlerle konur. Üst lobları etkileyen bronşektazi tanıda önemli bir kriterdir. ABPA beş klinik evrede değerlendirilir. Hastalığın şiddeti steroid bağımlı astım ve/veya yaygın fibrozisli bronşektazi ile yakından ilişkilidir. Sistemik steroidler tedavinin temelini oluşturur, ancak son yıllarda itrakonazolün de ABPA tedavisinde etkili olduğu gösterilmiştir.

Abstract

Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity reaction occurring in sensitive patients after the colonization of Aspergillus spores in bronchial secretions. Aspergillus fumigatus is involved in the majority of ABPA cases. The disease is mostly seen in patients with persistent asthma or cystic fibrosis. Diagnosis of ABPA is based on clinical, radiological and serological criteria. Bronchiectasis in the upper lobes is an important criteria for the diagnosis. ABPA is observed in five clinical stages. The severity of the disease is related to steroid dependent asthma or/and diffuse bronchiectasis with fibrosis. Treatment is based on systemic steroid therapy. However, in the recent years it was shown that itraco-nazole is also effective in the treatment.
