Tüberküloz plörezide gen-prob amplifiye Mycobacterium tuberculosis direkt test yönteminin tanı değeri

Özet
Tüberküloz (TB) plörezi tanısı balgam, plevral sıvı veya plevra biyopsi örneğinde TB basilinin belirlenmesi ya da histopatolojik inceleme ile kazeöz granülomların gösterilmesiyledir. Plevra biyopsisi travmatik bir işlem olup, çoğu olguda tanısal değildir. Teksif ile basilin saptanma oranı düşüktür, kültür içinse altı-sekiz hafta gerekir. Çabuk sonuç vermesi nedeniyle Amplifiye Mycobacterium tuberculosis Direkt Test (AMTDT) 40 TB plörezili, 40 kontrol grubundan 80 hastada çalışıldı. TB plörezide %20 sensitivite, %92.5 spesifisite, %72.7 pozitif prediktif değer, %46.3 negatif prediktif değer bulundu. Sonuç olarak düşük sensitivite, düşük spesifisite değerleri ve yüksek maliyeti nedeniyle TB plörezide rutin kullanılacak bir test olmadığını düşünüyoruz.
Abstract
Tuberculous pleurisy is diagnosed with AFB smear test in sputum, pleural fluid, and pleural biopsy specimens or with his-topathological examination. Pleural biopsy is a traumatic procedure and not diagnostic in most cases. AFB smear positivity rate is low in pleura and culturing needs 6-8 weeks. Since it's a rapid test, Amplified Mycobacterium tuberculosis Direct Test (AMTDT) was studied in 40 patients with tuberculous pleurisy and in 40 healthy people. In tuberculous pleurisy sensitivity and specifity were 20% and 92.5%, respectively, and positive predictive value was found 72.7%, negative predictive value was found 46.3%. As a result, because of its low specificity, sensitivity and higher costs it's not recommended as a routine test in tuberculous pleurisy.
