Özet
Cerrahi rezeksiyon küçük hücreli dışı akciğer kanserli (KHDAK) olgularda en iyi tedavi seçeneğidir. Komplet rezeksiyona rağmen hastaların çoğunda nüks olmaktadır. Bu durum KHDAK'li hastaları adjuvan kemoterapiye aday haline getirir. Postoperatif radyoterapi (PORT) Meta-Analiz Çalışma Grubu 1998 yılında adjuvan radyoterapi ile ilgili ilk meta-analizi yayınlamıştır. Araştırmacılar PORT'un komplet rezeksiyon geçirmiş evre I-II olgularda zararlı etkisi olduğu, N2 hastalarda ise rolünün belirsiz olduğu sonucuna varmışlardır. PORT Meta-Analiz Çalışma Grubu analizlerini yenilemişler fakat sonuçlar değişmemiştir. Kemoterapinin de adjuvan tedavide yeri araştırılmaktadır. 1990'lı yılların ortalarında yapılan adjuvan kemoterapi meta-analizinde beş yıl sağkalımda %5 avantaj gösterilmesinden sonra birçok yeni çalışma başlamıştır. Bunlardan Uluslararası Adjuvan Akciğer Kanser Çalışma (IALT) Grubu çalışması rezeke akciğer kanserli olgularda postoperatif kemoterapinin yararını konfirme etmiştir. Daha yeni, platin içeren modern kemoterapi çalışmaları da adjuvan kemoterapinin sağkalıma katkısını göstermiştir. Bu makalede adjuvan tedavide son durum gözden geçirilecektir.
Abstract
Surgical resection is the best treatment choice for patients with non small-cell lung cancer (NSCLC). However, the vast majority of patients will eventually relapsed, despite complete resection. This condition makes the patients potential candidate for effective adjuvant therapy. The Postoperative Radiotherapy (PORT) Meta-Analysis Trialists Group published initial meta-analysis in 1998. They concluded that PORT was deprimental to patients who had completely resected stage I-II non small-cell lung cancer, but its role in patients who had resected N2 disease was unclear. The PORT Meta-Analysis Group recently updated their analysis but the overall conclusions were not altered. Chemotherapy has been also explored in the adjuvant setting. Several adjuvant trial were launched in the mid 1990s after a meta-analysis suggested a 5% survival benefit at five years. Among those the International Adjuvant Lung Cancer Trial (IALT) study confirmed the benefit of postoperative chemotherapy in resected non small-cell lung cancer. More recently modern platinium doublets also showed the survival benefit of adjuvant chemotherapy. In this article the current status of adjuvant therapy is rewieved.
