Özet
Çalışmamızda Mart 2003-Ağustos 2004 tarihleri arasında tanı koyduğumuz üç senkron primer akciğer kanseri olgusunu sunduk. Hastaların tümü erkek olup, yaş ortalaması 52 yıl idi. Tüm hastalar sigara içicisi idi (ortalama 59 paket/yıl). Tümör tipleri bir olguda skuamöz hücreli kanser (SHK)-küçük hücreli akciğer kanser (KHAK), bir olguda adenokarsinom-ade-nokarsinom, bir olguda adenokarsinom-SHK idi. Tüm olgularda tümöral lezyonlar iki farklı akciğerde yerleşmişti, sistemik ve mediastinal yayılım yoktu. Üç olgunun başvurusu sırasında direkt akciğer grafilerinde ikinci primer akciğer kanseri lezyonu görülmedi. Bunların ikisinde lezyon bilgisayarlı tomografide, diğerinde ise ilk primer kanser tanısından yedi ay sonra posteroanterior akciğer grafisinde görüldü. Tanı, bir olguda transtorasik "tru-cut" biyopsiyle, bronkoskopi, diğer olguda transtorasik "tru-cut" biyopsi, video yardımlı torakoskopik cerrahi + wedge biyopsi, diğer olguda bronkoskopi, mini torakotomi + wedge biyopsi ile konuldu. Her iki lezyon ayrı ayrı evrelendirildi. iki olguda cerrahi uygulandı. Diğer olguda cerrahi + KHAK varlığı için sistemik kemoterapi yapıldı. Olguların şu anda üçü de hayattadır. Yalnız KHAK tanısı konan olgu tam remisyon olarak değerlendirildikten dört ay sonra nüks etmiştir. Olgularımızın prognozu göz önüne alındığında, birden çok akciğer lezyonu olan hastalarda, primer senkron akciğer kanseri olasılığı akılda tutularak, lezyonların herbirinden ayrı ayrı biyopsi yapılması uygun olmakta, bu yaklaşım ile olgulara cerrahi tedavi şansı verilmekte ve medikal tedavi protokollerine göre daha uzun sağkalım sağlanabilmektedir.
Abstract
The presence of multiple simultaneous pulmonary tumors in different locations is called synchronous lung tumor. Its incidence is 0.26-1.33%. We presented three synchronous primary lung cancer cases diagnosed between March 2003-Au-gust 2004. The patients were male smokers (mean 59 pack-years) with a mean age of 52 years. Tumor types were squ-amous cell carcinoma (SCC)-small cell lung carcinoma (SCLC), adenocarcinoma-adenocarcinoma, adeocarcinoma-SCC. All cases demostrated bilateral-involvement without systemic or mediastinal metastases. The second lesion was not detected by chest X-ray in any of them in first visit. Computerized tomography was demostrative for the lesion in the other. Diagnoses were made by transthorasic tru-cut biopsy and bronchoscopy in one, transthorasic tru-cut biopsy and video assisted thoracoscopic surgery + wedge biopsy in the other and bronchoscopy and mini thoracotomy + wedge biopsy in the another. Both of the lesions were staged independently. Two cases underwent surgery, whereas the other re-cieved systemic chemotherapy for SCLC in addition to surgery. All three patients are still alive. SCLC was found out to relapse four months after complete remission. Keeping in mind the possibilty of primary synchronous lung carcinoma, multiple lung tumors should be seperately biopsied. This approach enables the chance of surgical treatments and longer survival, compared to medical treatment protocols.
