Özet
Kırk yaşında bayan hasta, üç aydır devam eden efor dispnesi ile başvurdu. Fizik muayenede sol hemitoraks alt bölgesinde matite ve solunum seslerinde azalma saptandı. Rutin laboratuvar incelemeleri normal bulundu. Akciğer grafisinde sol sinüs kapalılığı ve alt mediastende genişleme izleniyordu. Bilgisayarlı toraks tomografisinde ön mediastenden köken alan, sol hemitoraksın orta-alt kısmını tamamen dolduran, yağ içerikli dev bir kitle saptandı. Radyolojik olarak lipom ya da ti-molipoma benzeyen lezyonun, sol alt lobu kompresyona uğrattığı raporlandı. Fiberoptik bronkoskopide lezyon saptanmadı. Patolojik tanının doğru bir şekilde konulabilmesi için büyük örneklere gerek duyulması ve cerrahi tedavinin gerekli olması nedeniyle sol torakotomi yapıldı. Ön mediastenden köken alan, sol alt lobu dolduran kapsüllü kitle tam olarak çıkarıldı. Sarı renkli rezeksiyon materyalinin ağırlığı 1970 g, boyutları 25 x 20 x 12 cm idi. Histopatolojik inceleme, timolipoma tanısını kesinleştirdi. Olgu, ilginç görünümü ve nadir olması nedeniyle sunuldu.
Abstract
A forty-year old female patient attended to the hospital with exertional dyspnea lasting three months. Her physical examination revealed dullness and decrease in the breath sounds on the basis of the left hemi-thorax. All the routine laboratory test results were within normal range. Her chest radiograph revealed an elevation of the left hemi-diaphragm and inferior mediastinal enlargement. A gigantic mass lesion with a fat attenuation coefficient, originating from the anterior mediastinum and involving the middle and the lower parts of the left hemi-thorax was detected on the computerized tomography of the thorax. The lesion that resembled lipoma or thymolipoma was reported to compress the left lower lobe. No lesion was detected by fiberoptic bronchoscopy. As large samples are needed for accurate histbpathological diagnosis and surgical treatment is essential, left thoracotomy was performed. An encapsulated tumour originating from the anterior mediastinum and involving the left lower lobe was totally resected. The yellowish resection material was 1970 g with 25 x 20 x 12 cm diameters. Histopathological examination confirmed the diagnosis of thymolipoma. The case is presented as it is a rare and interesting entity.
