Özet

Fibromatozlar (Desmoid Tümörler) tüm vücutta görülebilen, muskuloaponeurotik yapılardan kaynaklanan, benign fibröz neoplazmalardır. Bu tümör infiltratif büyüme paterni nedeniyle agresif olarak kabul edilmiştir ancak metastaz yapmamaktadır. Fibromatozis kısmen iyi bilinen bir tümördür ve sıklıkla tüm vücudu içeren vakaları bildirilmektedir. Ancak özellikle mandibulayı tutan oral kavitenin fibromatozisleri çok nadir olarak görülmektedir. 64 yaşındaki kadın bir hasta, mandibulanın sağ alveolar bölgesini içeren ve giderek büyüyen bir ağrısız kitle şikayetiyle kliniğimize başvurdu. Tümör, geniş marjinli bir lokal eksizyon ile alındı. 24 aylık bir cerrahi sonrası takip periyodu sonunda rekürrens görülmedi. Oral kavitede nadir görülen bu tür lezyonlarda en etkili tedavi yöntemi, olası bir rekürrensi de göz önünde bulundurarak geniş marjinli tam bir cerrahi eksizyondur. Bunun yanı sıra gingival fibromatozisler haricinde oral kavitede oluşan bu tümörlerin her zaman çene kemiklerine invaze olabileceği akılda tutulmalıdır.

Abstract

Fibromatoses (Desmoid tumors) are histologically benign fibrous neoplasms arising from the musculoaponeurotic structures throughout the body. This tumor has been considered locally aggressive because of the infiltrative growth pattern, but it does not metastasize. Fibromatosis is a relatively well-known and frequently reported entity when cases involving the total body are considered; however, fibromatosis of the oral cavity is uncommon and is even more rare when it involves the mandible. A 64-year-female patient was referred to our clinic because of increasingly growing painless mass which involved the entire right alveolar ridge of the mandible. Local excision with wide margins of the tumor was performed. The patient was followed-up and showed no signs of recurrence 24 months post-surgery. The most effective treatment method of these rare lesions of the oral cavity is complete surgical excision with wide margins considering the possibility of recurrence. Additionally, it should always be kept in mind that these lesions formed in the oral cavity always invade the jaw bones except gingival fibromatoses.
