Farkl fiber yerletirme tekniklerinin endodontik tedavi görmü molar dilerin krlma dayanm üzerine etkisi

Özet
Amaç: Bu çalmann amac farkl fiber yerletirme tekniklerinin endodontik olarak tedavi edilmi molar dilerin basnç karsnda krlma dayanm üzerine olan etkilerini incelemektir. Gereç ve Yöntem: Yeni çekilmi altm adet mandibular insan molar dii dört gruba ayrld (n=15). Birinci gruptaki dilere herhangi bir preparasyon ilemi uygulanmad (kontrol). Grup 2-4 deki dilere MOD kavite preparasyonu ve kanal tedavisi yapld. Daha sonra Grup 2 deki diler sadece kompozit rezin (KR) ile restore edildi. Grup 3 de diin KR ile restorasyonundan önce polietilen ribbon fiber bukko-lingual yönde kavite tabanna yerletirildi. Grup 4 de KR ile diin restorasyonu srasnda polietilen ribbon fiber bukkolingual yönde diin geliimsel oluklarnn içine yerletirildi. Örnekler nstron cihaznda, ucunda paslanmaz çelik bilye bulunan özel bir uç diin okluzal yüzeyine dik olacak ekilde 1mm/dk hzda basma kuvvetine maruz brakld. Krlma de- erleri Newton cinsinden kaydedildi. Veriler Tek Yönlü ANOVA ve Tukeys Çoklu Karlatrma testleriyle analiz edildi (=0.05). Bulgular: Elde edilen ortalama basnç dayanm deerleri: 1.Grup: 2485.30 ± 193.90N, 2.Grup: 1748.20 ± 510.00N, 3.Grup: 1728.67 ± 503.44N, 4.Grup: 1866.73 ± 399.83N. Kontrol grubu, dier restorasyon tekniklerinden istatistiksel olarak daha yüksek basnç dayanm gösterdi. Fakat, test edilen restorasyon teknikleri arasnda istatistiksel bir fark bulunamad (p>0.05). Krlma tipleri incelendiinde, grup 4de krlmalarn çou sadece kompozit rezinde oluurken, dier gruplarda genellikle dite ve mine sement snrna uzanan krlmalar gözlendi. Sonuç: Test edilen restorasyon tekniklerinin hiç birinde dilerin basnca dayanm salam diin basnç dayanmna ulaamad. Fakat KR ile diin restorasyonu srasnda fiberin bukko- lingual yönde geliimsel oluklarn içine yerletirilmesi tekniinde restorasyonun tamirine izin veren k- rklar olutuu için bu tekniin daha güvenilir olabilecei düünüldü.
Abstract
Background: The aim of the study was to investigate the effect of different fiber placement techniques on fracture resistance of endodontically-treated molars. Methods: Sixty freshly extracted human mandibular molars were assigned to four groups (n=15). Group 1, did not receive any preparation. The teeth in Groups 24 received MOD cavity preparation and root canal treatment. In group 2, teeth were restored with a composite resin (CR). In group 3, polyethylene ribbon fiber was inserted in to cavity floor in buccal-to-lingual direction before the restoration of the teeth with CR. In group 4, polyethylene ribbon fiber was inserted into the groove in buccal to lingual direction during the restoration of teeth with CR. After finishing, polishing and thermalcycling, the specimens were subjected to compressive loading with a special stainless steel point perpendicular to the occlusal surface at crosshead speed of 1 mm/min. The loads necessary to fracture were recorded in Newton. The results were analyzed with one way ANOVA and Tukeys tests. Results: The mean fracture values were as follows: Group 1: 2485.30 ± 193.90, Group 2: 1748.20± 510.00, Group 3: 1728.67± 503.44, Group 4: 1866.73± 399.83. No statistically significant differences were found between tested restoration techniques (p>0.05). However, most of the fractures in group 4 formed only in composite resin while in other groups fractures showed generally at teeth and reached over to cemento-enamel junction. Conclusions: None of the tested restoration techniques was able to completely restore the fracture resistance of teeth. However insertion of polyethylene ribbon fiber into the groove in buccal to lingual direction seems more reliable than other techniques for allow to repairing of fractured restorations.
