Two-year clinical evaluation of yttria-stabilized tetragonal zirconia polycrystalline ceramic and titanium abutments and single crowns

Özet
Amaç: Bu çalmann amac titanyum implantlar üzerinde kullanlan Yttriyum ile desteklenmi tetragonal zirkonyum oksit polikristalin (Y-TZP) seramik ve titanyum abutmentlar 2 yl boyunca deerlendirmektir. Gereç ve yöntemler: 21 hastada toplam 48 implant destekli kron çalma kapsamna alnmtr. Tüm implantlarda standart iki aamal cerrahi prosedür uygulanmtr. Hazrlanan kronlar Y-TZP (ZC) ve metal alt yapl porselen (PFM) kronlar olmak üzere iki gruptur. Yükleme yapldnda ve yükleme yapldktan 6, 12 ve 24 ay sonra gerçekletirilen takip seanslarnda implant yaplar incelenmitir. Her randevuda, implant üstü restorasyonlar kron ve abutmentlarda krlma ve abutment vidasnn kayb yönünden deerlendirilmitir. Bunun dnda implant mobilitesine (evet/hayr) ve implant-kron birleimindeki plak birikimine de baklmtr. Bulgular: Titanyum abutment üzerindeki iki adet metal alt yapl porselen kronda ve Y-TZP abutment üzerindeki bir adet zirkonyum alt yapl kronda olmak üzere toplam 3 adet abutment vidasnn kayb gözlenmitir. Abutment vidasnn kayb açsndan gruplar arasnda istatiksel olarak anlaml bir fark bulunmamtr (p>0.05). Tüm gruplarda herhangi bir abutment ve kron kr gözlenmemitir. Zirkonyum ve metal alt yapl porselen kronlarn hiçbirinin üst yap seramiinde bir krlma ya da atma olmamtr. 6. ve 12. aydaki kontrol randevularnda, plak birikimi için 1 ve 2 eklinde skor deerleri verilmitir. Plak birikimi açsndan gruplar arasnda istatiksel olarak anlaml bir fark bulunmamtr (p>0.05). Sonuç: 2 yllk süreçte, Y-TZP abutmentlar, titanyum abutmentlarla ayn baar orann göstermilerdir. Abstract
Background: The aim of this study was to evaluate yttria-stabilized tetragonal zirconia polycrystalline ceramics (Y-TZP) and titanium abutments on titanium implants up to 2 years. Methods: Twenty-one patients who were treated with 48 implant supported crowns were analysed. For all implants, a standardized twostage surgical procedure was used. Y-TZP (ZC) and porcelain-fusedto-metal (PFM) crowns were fabricated. The follow-up examinations comprised clinical assesment of the implant reconstructions and were performed at baseline, 6, 12, and 24 months of loading. Fracture of abutments, crowns and abutment screew loosening were evaluated for each clinical appointment. Mobility of the implant (yes or no), amount of plaque at abutment-crown combinations were also evaluated. Results: Abutment screw loosening occurred at two PFM crowns on titanium abutments and at 1 ZC crown on Y-TZP abutment. There were no statistically significant differences among groups in respect to abutment screw loosening (p>0.05). Fracture of an abutment and crown was not observed in all groups. No chipping of the veneering ceramic occurred at ZC and PFM crowns. At 6 and 12 month recall appointments, score 1 and 2 for plaque accumulation were observed. There was no statistically significant difference among groups in respect to plaque accumulation (p>0.05). Conclusions: Y-TZP abutments exhibited the same success rate as titanium abutments during a 2-year period.
