Cinsel Saldırı sonucunda gebe kalan olguların değerlendirilmesi

Özet

Cinsel saldırı sonucu gebe kalan olgular adli tıbbın önemli bir konusu olup; bu araştırmada 2011-2013 yılları arasında Hacettepe Üniversitesi Tıp Fakültesi Adli Tıp Anabilim Dalı’na cinsel saldırı sonucu beden ve ruh sağlığının değerlendirmesi amacıyla başvuran gebe olguların özellikleri irdelenmiştir. 2011-2013 yılları arasında toplam 23 cinsel saldırı sonucu gebe kalan kadının beden ve ruh sağlığının değerlendirilmesi amacıyla başvurmuş olduğu, olay tarihindeki yaşlarının 13-34 aralığında olduğu, %70’inin olay tarihinde 18 yaşından küçük olduğu, % 65 vakada olguda cinsel saldırının mükerrer olduğu, 11 olguda zeka geriliği tespit edildiği ve olguların tamamının saldırganı tanıyor olduğu tespit edilmiştir. Cinsel saldırı sonucu oluşan gebelikler önlenebilir olmasına rağmen geç fark edilmesi, cinsel saldırı mağdurlarının başvurabilecekleri merkezlerin bulunmaması, yeterince destek alamamaları ve toplumsal baskılar gibi nedenlerle çoğunlukla ileri evrede saptanmakta, ileri evrede olmadığı durumlarda bile pek çok farklı nedenle gebelik sonlandırılamayabilmekte ve mağdur doğum yapmak zorunda kalabilmektedir. Cinsel saldırı sonrası mağdurun bu tür istenmeyen gebelikten korunması için mutlaka saldırı sonrası başvurularda acil kontrasepsiyon uygulanmalı, gebeliğin erken evrede saptandığı olgularda mağdurun rızası ile tıbbi abortus düşünülmelidir. İleri evrede saptanan gebelerde ise mağdurun rızası ile annenin sağlığını tehdit etmediği sürece Adli Tıp, Psikiyatri ve Kadın Hastalıkları ve Doğum Uzmanlarından oluşan bir heyet tarafından olgu ele alınarak, olgu özelinde karar alınmalıdır.

Abstract

Becoming pregnant as a result of sexual assault, is an important subject of forensic medicine. In this study, the characteristics of sexual assault related pregnancy cases, who admitted to Hacettepe University Faculty of Medicine Department of Forensic Medicine for evaluation of physical and mental health were studied. It was found that totally 23 cases who became pregnant due to a sexual assault admitted for physical and mental health evaluation between years 2011 and 2013, age of the victim at the time of the incident ranged 13 to 34, 70% were under 18 years of age at the time of the incident, 65% of the cases were repetitive, 11 had mental retardation and all of the victims knew the assailant before. Although the pregnancy related to sexual assault is a problem that can be prevented, they are mostly determined in a late period of pregnancy for some reasons such as late recognition, the lack of sexual assault centers that the victims may apply, lack of enough support and social pressure. Even in the situations out of the late periods, the pregnancy may not be terminated for a number of different reasons and the victim has to make the birth. Emergency contraception must be applied at the admission after a sexual assault in order to protect the victim from those unwanted pregnancies. In the earlier period of the pregnancy, medical abortion should be thought with the consent of the victim. For the pregnancies that were detected within the late period, case by case evaluation should be made by a board which consists of forensic medicine specialists, psychiatrists and obstetricians while the victim gives consent and as long as it doesn’t threaten the health of her.
