Özet
Giriş: Sağlık hakkı, en temel insan haklarının başında gelir (İnsan Hakları Evrensel Bildirgesi madde 25). Kaynaklar sağlık sektöründe, öbür sektörlerde olduğu gibi sınırlıdır. Bu çalışmada amaç, sosyalleştirmeden sağlıkta dönüşüme Türkiyedeki sağlık harcamalarını öbür ülke örnekleriyle karşılaştırarak incelemektir. Gereç ve Yöntem: Bu araştırma, Türkiyede sağlık harcamalarını 1961den başlayarak günümüze dek inceleyen retrospektif boyutlu tanımlayıcı bir çalışmadır. Bulgular: Sosyalleştirmeden günümüze toplam sağlık harcamaları sürekli olarak, 2002 yılı ve sonrası ise Gayri Safi Yurt İçi Hasıla artış hızını aşarak büyümüştür. Ayakta sağaltım hizmetlerinde özel sektör cari sağlık harcamaları, kamunun 2,3 katına ulaşmıştır. 1997de tedavi hizmetlerinin Sağlık Bakanlığı bütçesine oranı %39,2 ile temel sağlık hizmetlerinin oranına (%38,8) yakınken, 2006 yılında bu oran sırasıyla %66,7ye karşılık %24,3tür. 2011de Sosyal Güvenlik Kurumunun sağlık harcamalarının %38,8ini ilaç gideri oluşturmuştur. Türkiyede 2001den 2007ye dek eşdeğer ilaç pazarı %30 düzeylerinde kalmış olup; belirgin artış görülmemiştir. 2010-2012 döneminde eşdeğer ilaç pazarı Türkiyede %40lara yaklaşmıştır. Sonuç: Türkiye, kamusal ağırlıklı ulusal bir sağlık sistemini benimsemelidir. Sağlık harcamalarını azaltmak için koruyucu sağlık hizmetlerine öncelik verilmelidir. İlaç giderlerindeki gereksiz artışı önlemek için eşdeğer (jenerik) ilaçların yazımı ve temel ilaçların akılcı kullanımı sağlanmalıdır.
Abstract
ntroduction: Right to health is one of the leading basic human rights (The Universal Declaration of Human Rights, article 25). Resources are limited in the health sector as well as other sectors. The aim of this study is to analyze the health expenditures in Turkey from socialization of health services to health reforms, in comparison to other country examples Material and Method: This study is a descriptive study with retrospective dimensions, analyzing the health expenditures in Turkey from 1961 up to the present time. Results : Health expenditures have increased continuously and rapidly in time and tend to increase faster than the growing rates of the gross domestic product since 2002. Currently private health expenditures are 2.3 times more than the public services at out-patient health care. The percentage of the expenditures of the curative services to the health ministry budget was 39.2% in 1997, close to the level of primary health care services, which was 38.8%. The same percentages were 66.7% and 24.3% respectively in 2006. Drug expenditures were 38.8% of the total Social Security Institution health expenditures in 2011. In Turkey, the generic drug market has remained at the 30% levels with no conspicuous increases from 2001 to 2007. However, the generic market has approached 40% during the 2010-2012 period. Conclusion: Turkey should adopt a national health system which is based on public services. For decreasing health expenditures, preventive health services should be prioritized. Generic drug prescribing and rational use of essential drugs must be provided for avoiding irrational increases in drug expenditures.
