Hepatit B Taşıyıcılarının Hastalık Hakkındaki Bilgi, Düşünce ve Tutumları

Özet
Yaygın bir sağlık sorunu olan hepatit B (HBV) enfeksiyonunun neden olduğu kronik hepatit, siroz ve hepatosellüler karsinoma (HSK)yol açarak milyonlarca ölüme neden olmaktadır. Bu enfeksiyonun sosyal, psikolojik ve ekonomik sonuçları da vardır. Hepatit B taşıyıcılarının durumunu çeşitli yönleriyle belirlemek amacıyla yapılan bu araştırmada yapılan HBsAg testi sonucu pozitif çıkan 130 taşıyıcı olgu çalışma grubunu oluşturdu. Yüzyüze görüşme yapılarak sosyodemografik özellikleri, bilgi düzeyi, tutumları araştırıldı; ELISA testi kayıtları incelendi. Serolojik durumları, hepatit B bulaşmasında etkili risk etmenleri değerlendirildi. Hepatit B hakkında bilgi verildi; bulaşma ve korunma yolları anlatıldı. Çalışmada bulunan HBsAg pozitifliği oranı %9.6, HBV seroprevalansı %41.1’dir. Diğer laboratuvar bulguları ile risk etmenleriyle karşılaşma durumları diğer çalışmalarla benzerdir. Hepatit B taşıyıcılarının hastalık hakkındaki bilgileri yetersiz ve yanlıştır. Taşıyıcı olduğu belirlenen kişilere hastalık hakkında sağlık çalışanları tarafından yeterli bilgi verilmediği, taşıyıcılar tarafından başkalarına hepatit B bulaşmaması için yeterli önlemler alınmadığı saptandı. Hastalığın tedavisi hakkında bilgiler yetersizdir ve yararsız, hatta zararlı yöntemler hastalarca tedavi amacıyla uygulanmaktadır. Taşıyıcılar aşılama yöntemine sıcak bakmaktadır, ancak gereksinimi olanların aşı maliyetini karşılama gücü yoktur. Sağlık kuruluşlarında hepatit belirteçleri incelenirken anti-HBs ve anti-HBc de bakılarak durumu uygun olanlar hepatit B’ye karşı aşılanmalıdır. HBsAg pozitifliği saptananlarla aile bireylerine ayrıntılı bilgi verilmesi, bulaşma yollarının ve alınacak önlemlerin anlatılması, taşıyıcıların düzenli izlenmesi gerekmektedir. Viral hepatitlilerin izleminde; biyokimyasal, serolojik ve radyolojik özelliklerin yanında psikolojik yönleri de değerlendirilip, desteklenmelidir. Enfeksiyonun yayılmasını önlemek için risk gruplarından başlayarak hepatit B aşılaması yaygınlaştırılmalıdır.
Abstract
Chronic hepatitis, cirrhosis and hepatocellular carcinoma (HCC) resulting from the hepatitis B (HBV) infection, which is a common health problem, cause millions of deaths. There are also social, psychological and economic consequences of this infection despite the decrease in its incidence. In this study that aimed at investigating the status of hepatitis B carriers in various aspects, the sample was composed of 130 carriers tested positive for HbsAg. Their sociodemographic characteristics, knowledge levels and attitudes were investigated through face-to-face interviews; ELISA test records were examined. The serological status and the risk factors having influence on hepatitis B transmission were evaluated. The information about hepatitis B was provided; the modes of transmission and prevention were explained. HBsAg positivity rate was found to be 9.6% and the seroprevalence of HBV was 41.1%. Other laboratory findings and status of exposure to risk factors were similar to those found in other studies. The knowledge of Hepatitis B carriers on the disease were insufficient and inaccurate. It was ascertained that the persons who have been confirmed to be carriers were not provided with sufficient information about the disease by the health workers and that the carriers did not take adequate precautions to prevent the transmission of hepatitis B to other ones. The information about the treatment of the disease was insufficient and useless, and even harmful methods were practiced by the patients for treatment. The carriers leaned towards the vaccination method, but those who needed vaccination could not meet the cost. Anti-HBs and anti-HBc should also be tested during the analysis of serological markers of hepatitis at health care facilities, and suitable ones should be vaccinated against hepatitis B. The persons, who are found to be HbsAgpositive and their family members should be informed in detail, the modes of transmission and the precautions to be taken should be explained and the carriers should be monitored regularly. During the follow-up of the persons with viral hepatitis; the psychological state should be evaluated as well as biochemical, serological and radiological aspects and the patients should be supported. In order to prevent the spread of infection, hepatitis B vaccination coverage should be improved starting from high-risk groups.
