Sigara alışkanlığı bronşial hiperreaktivite oluşturur mu?

Özet
Randomize seçilmiş 97 olguda bronşial hiperreaktivite (BHR) ve sigara alışkanlığı arasındaki ilişki araştırıldı. Yaş, cins, öksürük, balgam, nefes darlığı, hırıltılı solunum gibi solunum semptomları, sigara alışkanlığı ve günlük içilen~sigara sayısı kaydedilerek, tüm olgulara 1 mg/ml'den 25mg/ ml'ye kadar artan konsantrasyonlarda metakolin verilerek provokasyon testi uygulandı. Provokasyon sonucu FEVİ'de %20 düşme görülen olgular hiperreaktif, %20 düşme oluşturan konsantrasyon PC20 FEVİ kabul edildi. Bronşial hiperreaktivite sigara içen 51 olgunun 31'inde (%60), içmeyen 33 olgunun dokuzunda (%28) pozitif bulundu (p 0.05). Günlük içilen sigara sayısı ile BHR arasında ilişki kaydedildi. PC20 FEVİ değeri sigara içenlerde içmeyenlere göre daha düşük olmasına karşın, bu fark anlamlı bulunmadı. Sigara içenlerde bazal FEVİ ortalamaları içmeyenlere göre anlamlı şekilde düşük gözlendi (p<0.05). Sonuç olarak, sigara içiminin ve içilen sigara sayısının BHR'yi etkilediği sonucuna varıldı.
Abstract
We examined the relationship of cigarette smoking with bronchial hyper-reactivity (BHR) in 97 normal subjects. Relevant data were obtained in¬cluding age, sex, smoking habits, number of cigarettes consumed per day and respiratory symptoms such as cough, phlegm, dyspnea, and wheezing. Prov¬ocation test with metacholine was performed in increasing concentrations from 1 mg/ml to 25 mg/ml in all cases. Subjects in whom 20% decrease in FEV1 was observed were regarded as hyperreactive and the concentration leading to this decrease was accepted as PC20 FEV1. Bronchial hyper-reactivity was found positive in 31 (60%) of 51 smokers and in 9 (28%) of 33 nonsmokers (p<0.01). Respiratory symptoms in the smoker group were more prevalent (p<0.05). No statistically significant difference was found between females and males in the smoker group (p>0.05). The number of cigarettes smoked per day was found in correlation with BHR . PC20 FEV1 values of smokers was lower than those of nonsmokers but the difference was not sig¬nificant. Basal FEV1 values were significantly lower in smokers (p<0.05). We concluded that smoking and the number of cigarettes had influence on BHR.
