Hafif stabil astmalı olgularda periferik kan ve bronkoalveolar lavaj lenfosit altgruplarının bronş hiperreaktivitesi ve IgE ile ilişkisi

Özet
Astma havayolu inflamasyonu ve bronş aşırı cevaplılığı ile karakterize bir hastalıktır. Havayolunda kronik eozinof ilik inf lamasyonun oluşması ve de¬vamında aktif T lenfositleri önemli rol oynar. Çalışmada hafif stabil bronş astmalı olgularda periferik kan ve bronkoalveolar lavaj (BAL) örneklerinde lenfosit altgrupları flowsitometri ile değerlendirilerek PC20 metakolin (mg/ mL) ve serum total IgE düzeyi ile ilişkileri arandı. Yaş ortalaması 29.82±8.88 olan 17 atopik astmalı olgunun periferik kan lenfosit altgrupları yaş orta¬laması 28.29±10.04 olan 23 sağlıklı kişi ile karşılaştırıldı. Astmalı olgularda CD3, CD4 ve CD8'in periferik kanda, BAL'dan anlamlı derecede yüksek bulundu (p<0.05). Periferik kanda, kontrollere göre astmalı hastaların CD8 düzeyi daha yüksek saptandı (p<0.05). PC20 değeri ile periferik kan ve BAL lenfosit altgrupları arasında ilişki gösterilemedi. Serum total IgE düzeyi ile periferik kan CD25 ve CD4/CD8 oranı arasında çok zayıf bir ilişki olduğu bulundu.
Abstract
Asthma is a disease characterized by airway inflammation and bronchial hy-perresponsiveness. Activated T-lymphocytes play an important role in the development and progression of chronic eosinophilic inflammation in the airways. Peripheral blood and bronchoalveolar lavage fluid samples were obtained from 17 patients (mean age 29.82±8.88 years) with mild stable asth¬ma and lymphocyte subgroups were evaluated through flow cytometry. Cor¬relations were sought between lymphocyte subgroups and methacholine PC20 (mg/ml) and serum total IgE levels. Peripheral blood lymphocyte sub¬groups of the study patients were compared with those of 23 healthy control subjects with a mean age of 28.27±10.04 years. In asthmatic patients CD3, CD4, CD8 levels were significantly elevated in peripheral blood compared with those levels in BAL (p<0.05). Significantly increased CD8 levels were found in peripheral blood samples of asthmatics (p<0.05). There was no cor¬relation between PC20 values and lymphocyte subgroups of peripheral blood and BAL. We found a weak correlation between serum total IgE and pe¬ripheral blood CD25 levels and CD4/CD8 ratio.
