Özet
Alt ekstremitedeki iskemi-reperfüzyon hasarının oluşmasında basınç kontrollü, sınırlı reperfüzyonun etkisinin araştırılması amacıyla 19 köpekte üç grup üzerinde, sınırlı perfüzyon ve tam iskemi sonrası reperfüzyon çalışması gerçekleştirildi. Grup 1'de altı saat basınç kontrollü sınırlı perfüzyon sonrası, iki saat sistemik basınçla reperfüzyon uygulandı. Grup 2'de altı saatlik iskemi sonrası, iki saat sistemik basınçla reperfüzyon; grup 3'te ise altı saatlik iskemi sonrası, bir saat basınç kontrollü sınırlı reperfüzyonu takiben bir saat sistemik basınçla reperfüzyon uygulandı. Başlangıçta, klemp açılmadan önce, reperfüzyonun 15, 30, 60, 90, ve 120. dakikalarında LPO, LDH ve CPK ölçümleri için kan örnekleri alındı. Son kan örneği alındıktan sonra LPO tayini için gastrocnemius adalesinden parça alındı. Grup 2 ve 3'te LPO, CPK ve LDH değerlerinde anlamlı artışlar görüldü. LPO değerleri grup 2 ve 3'te 90. dakikada yüksekti. Grup l'de CPK ve LDH değerlerinde artış görüldü, ancak bu artışlar anlamlı değildi. Akut alt ekst-remite iskemisi sonrası erken dönemde basınç kontrollü sınırlı reperfüzyon uygulayarak, reperfüzyon hasarının azaltılabileceği sonucuna varıldı.
Abstract
In order to investigate the effect of pressure-controlled and limited reperfusion in ischemia-reperfusion damage of lower extremity, a study of reperfusion was carried out on 19 dogs divided into 3 groups after controlled perfusion and complete ischemia. In group 1, following six hours of controlled-pressure and limited perfusion, reperfusion was applied for two hours with systemic blood pressure. Group 2 underwent reperfusion for two hours under systemic blood pressure, following six hours of ischemia. Group 3, on the other hand, following six hours of ischemia underwent an hour of controlled pressure and limited reperfusion followed by two hours of reperfusion under systemic blood pressure. Blood samples were drawn to measure LPO, LDH, and CPK at the beginning, before unclamping, and at 15,30,60,90 and 120th minutes of reperfusion. After the last blood sample was drawn, a gastrocnemius muscle specimen was obtained to determine LPO. Significant increases in LPO, CPK and LDL values were found in groups 2 and 3. LPO values showed increase in group 2 at minute 90. Increases in CPK and LDH values in group 1 did not reach significance. We conclude that pressure-controlled and limited reperfusion in the early period may lessen reperfusion damage after acute ischemia of the lower extremity.
