Özet
Amaç: Bu çalışmada obezitede, insulin sekresyonunun oral ve intravenöz glukoz uyarısına göre değişikliklerinin gösterilmesi amaçlandı. Hastalar ve Yöntemler: Obez olgulara (n=17) ve kontrol grubu olarak alman obez olmayan 20 kişiye hem oral glukoz tolerans testi (OGTT), hem de intravenöz glukoz tolerans testi (IVGTT) uygulandı ve insulin cevabı incelendi. Obezite tespiti vücut kitle indeksi ile yapıldı. Her iki glukoz tolerans testi için insülinojenik indeksler hesaplandı. İstatistiksel değerlendirmede Student t-testi kullanıldı. Bulgular: Obez grupta OGTT ile glukoz intoleransı oranı %41, IVGTT ile diabetik olma oranı %6 iken bu oranlar kontrol grubunda her ikisi için de %5 bulundu. OGTT ve IVGTT ile ölçülen glisemi ve insülinemi değerleri obez grupta daha yüksek seyretti. Her iki glukoz tolerans test yönteminde, insülinojenik indeksler obez ve kontrol gruplarında anlamlı farklılık göstermedi (p=0.4). Sonuç: Obez hastalarda gözlenen glukoz intoleransının pankreasın insulin cevabındaki bir bozukluğa değil, insulin direncine bağlı olduğu sonucuna varıldı.
Abstract
Objectives: This study was designed to determine the alterations in insulin secretion in response to oral and intravenous glucose administration. Patients and Methods: To evaluate the insulin secretion, oral glucose tolerance test (OGTT) and the intravenous glucose tolerance test (IVGTT) were performed in 17 obese patients and 20 non-obese controls. Obesity was determined by means of body mass index. Insulinogenic indexes were calculated for both glucose tolerance tests. Statistical evaluation was made using the Student's t-test. Results: In the obese group, glucose intolerance ratio was 41% with OGTT, and diabetic ratio was 6% with IVGTT, whereas both ratios were 5% in the control group. Values for glycemia and insulinemia were found to be higher in the obese group. Insulinogenic indexes did not show significant differences in both glucose tolerance tests between the obese and control groups (p=0.4). Conclusion: We concluded that glucose intolerance observed in obese patients was not due to a disturbance jn insulin secretion of the pancreas, but to the insulin resistance.
