Romatoid artritli hastalarda siklosporin A tedavisi ile alınan sonuçlar

Özet
Amaç: Siklosporin A'nın romatoid artrit üzerindeki etkinliğini araştırmak.Hastalar ve Yöntemler: Çalışmaya romatoid artrit tanısı konmuş 23 hasta (16 erkek, 7 kadın; yaş ört. 43.6±16.8) alındı. Siklosporin A tedavisine 3 mg/kg/ gün ile başlandı ve hastalar ilk iki hafta haftalık, daha sonra ise aylık kontrollerle izlendi. Her kontrolde sabah sertliği süresi, görsel ağrı skalası, Lee ve Ritchie indeksleri ve şiş-hassas eklem sayıları gibi parametreler ve gözlenen yan etkiler değerlendirildi. Bulgular: Toplam 17 hastada (%73.9) ilaca bağlı yan etkiler görüldü. Bir hasta bronşiyal astım krizi, bir hasta şiddetli gastrointestinal semptomlar, bir hasta etkisizlik ve hipertrikozis, bir hasta da etkisizlik nedeniyle olmak üzere, dört hasta (%17.4) çalışmayı tamamlayamadı. Hastalarda tedavi sonrası Ritchie (p<0.001) ve Lee (p<0.001) indeksleri, görsel ağrı skalası (p<0.001), sabah sertliği süresi (p<0.05) ve hassas ve şiş eklemdeki (p<0.001) düzelmeler anlamlı bulundu. Sonuç: Siklosporin A'nın romatoid artritli hastalarda etkin ve faydalı olduğu görülmektedir. Bununla birlikte, yüksek orandaki yan etki profili göz önüne alınmalıdır.
Abstract
Objectives: To evaluate the efficacy of cyclosporin A in rheumatoid arthritis. Patients and Methods: The study included 23 patients (16 males, 7 females; mean age 43.6±16.8 years) with rheumatoid arthritis. Cyclosporin A 3 mg/kg was administered daily. The patients were examined weekly for two weeks and then montly thereafter, during which morning stiffness, pain by visual analogue scale, Lee and Ritchie indices and number of tender-swolen joints were evaluated, together with adverse effects. Results: Drug-induced adverse effects were observed in 17 patients (73.9%). Therapy was discontinued in four patients (17.4%) due to the occurrence of asthma attacks (n=1), severe gastrointestinal symptoms (n=1), inefficiency and hypertrichosis (n=1), and inefficiency alone. Significant posttreatment improvement was observed in Rithcie (p<0.001) and Lee (p<0.001) indices, visual analogue scale (p<0.001), morning stiffness (p<0.05), and tender or swollen joints (p<0.001). Conclusion: Cyclosporin A appears to be efficacious and useful in patients with rheumatoid arthritis; however, considerable account should be given to its adverse effects.
