Orofarenjeal topikal anestezinin sağlıklı kişilerde solunum fonksiyon testlerine etkisi

Özet
Amaç: Rutin KBB pratiğinde kullanılan topikal anestezinin solunum fonksiyon testi (SFT) parametrelerine etkileri araştırıldı.Çalışma Planı: Bilinen bir alt ve üst solunum yolları hastalığı anemnezi bulunmayan 15 sağlıklı erişkin, gönüllü (3 kadın, 12 erkek) çalışmaya alındı. Deneklerin orofarenjeal alanına beş farklı noktaya topikal anestezik (lidokain %10 sprey, 10 mg/puff, toplam 50 mg) pulverize edildi. İşlem öncesi ve sonrası 10. ve 60. dakikalarda SFT uygulandı. İstatistik analizde Wilcoxon İkili Eşleştirme testi kullanıldı.Bulgular: Tüm deneklerde belirgin yutkunma güçlüğü, hava alamama hissi, boğaz temizleme çabası gözlendi. Larenjeal muayenede priform sinüslerde ve dil kökünde hafif tükürük göllenmesi dışında objektif bulgu saptanmadı. Solunum fonksiyon testi parametrelerinde anlamlı değişiklik oluşmadı (p>0.05).Sonuç: Bazı gönüllülerde inspiratuar parametrelerde değişiklikler görülmekle birlikte, bunlar grup genelinde istatistiksel anlam taşımamaktadır. Sübjektif yakınmalarda psikolojik komponentin de etkili olabileceği düşünüldü.
Abstract
Objectives: We evaluated the effects of oropharyngeal anesthesia on respiratory function test (RFT) parameters. Study Design: The study included 15 healthy adult volunteers (3 females, 12 males) without any known history of upper and/or lower respiratory diseases. A topical anesthetic (lidocaine 10% spray, 10 mg/puff, 50 mg total) was applied to five different locations in the oropharyngeal region. Respiratory function tests were performed prior to, in the 10th, and 60th minutes after, the procedure. Statistical analyses were made using Wilcoxon matched-pairs test. Results: All subjects exhibited difficulty in swallowing, attempts to take deep breath, and consequent attempts to clear throat. No objective findings, but mild saliva pooling in the pyriform sinus region were found. Respiratory function test results showed no significant changes (p>0.05).Conclusion: Although some changes were detected on inspiratory parameters in some subjects they did not reach a statistical value. We concluded that psychologic components may also play part in the development of subjective complaints.
