Tonsillektominin serum IgG, IgM, IgA, $C_3$ ve $C_4$ düzeylerine etkisi

Özet
Amaç: Tonsillektominin serum immunglobulinler IgG, IgM, IgA ve kompleman sistemin faktörleri olan C3 C4 düzeyleri üzerine etkisini araştırmak. Hastalar ve Yöntemler: Kronik tonsillit tanısıyla 17 hastaya (9 erkek, 8 kız; ort. yaş 7; dağılım 5-12) tonsillektomi uygulandı. Olgulardan ameliyattan bir gün önce (grup A) ve bir ay sonra (grup B) olmak üzere iki kez kan örnekleri alındı. İmmünglobulin ve kompleman faktör düzeyleri nefelometrik olarak ölçüldü. Sonuçlar bildirilen normal değerlerle karşılaştırıldı. Bulgular: Grup A'da IgG ve IgA düzeyleri grup B ve normal değerlere göre anlamlı derecede yüksekti (p<0.05). Bu değerlerin tonsillektomi ile normal sınırlar içine indiği gözlendi. IgM, C3 ve C4 düzeylerinde ise anlamlı bir değişiklik saptanmadı. Grup B ile normal değerler arasında da anlamlı bir fark görülmedi. Sonuç: Çalışmamız, tonsillerin kronik enfeksiyona maruz kalma sürecinde özellikle IgG ve IgA ürettiğini, IgM ve kompleman sisteminin bundan fazla etkilenmediğini ve tonsillektominin IgG ve IgA üretimini normal seviyeye düşürdüğünü gösterdi.
Abstract
Objectives: To investigate the effect of tonsillectomy on serum levels of immunoglobulins G, M, A and complement factors C3 and C4. Patients and Methods: Seventeen patients (9 boys, 8 girls; mean age 7 years; range 5-12 years) with chronic tonsillitis underwent tonsillectomy. Blood samples were obtained one day before (group A) and one month after surgery (group B). Serum immunoglobulin and complement factor levels were measured by nephlometry and compared with normal ranges. Results: In group A, IgG and IgA levels were significantly higher than those in Group B and normal ranges (p<0.05). These values returned to normal levels after tonsillectomy. No significant differences were detected in IgM, C3 and C4 levels. Similarly, no significant differences were observed between group B values and the normal ones. Conclusion: The study showed that the tonsils produced increased IgG and IgA during exposure to chronic infection, without changing normal ranges of IgM, C3 and C4, and that IgG and IgA return to normal ranges following tonsillectomy.
