Perkütan trakeostomi uygulamalarımızın cerrahi teknikle karşılaştırılması

Özet
Amaç: Çalışmamızda perkütan trakeostomi (PT) ile cerrahi trakeostomiyi (CT), uygulama sırasındaki komplikasyonlar, hazırlanma süresi, işlem süresi ve toplam süreyi değerlendirerek karşılaştırmayı amaçladık. Hastalar ve Yöntemler: Çalışma, prospektif olarak yoğun bakım servisinde yatmakta olan 51 hastaya yatakları başında yapıldı. Rastgele seçimle 25 hastaya PT (16 erkek, 9 kadın; ort. yaş 44.2; dağılım 21-53), 26 hastaya CT (18 erkek, 8 kadın; ört. yaş 46.1; dağılım 24-56) uygulandı. Bulgular: İki grupta gelişen komplikasyonlar hafif ve önemsiz olup gruplar arasında fark bulunamadı. Girişim öncesi hazırlık süresi PT'de 19.0±6.3 dk, CT'de 28.7±6.9 dk (p<0.01); işlem süresi PT'de 6.8±3.2 dk, CT'de 18.3±4.1 dk (p<0.01); toplam süre PT'de 32.1 ±11.2 dk, CT'de 56.2±10.7 dk (p<0.001) bulundu. Sonuç: Perkütan trakeostominin hasta yatağı başında uygulanabilen emniyetli ve basit bir yöntem olduğu, hazırlık ve işlem süresi olarak kısa sürdüğü ve daha az elemana ihtiyaç gösterdiği sonucuna varıldı.
Abstract
Objectives: We compared percutaneous tracheostomy (PT) and surgical tracheostomy (ST) with regard to duration for preparation, duration of procedure, overall duration of procedure, and intraoperative complications. Patients and Methods: This prospective, randomised study was performed in 51 patients at bedside in the intensive care unit. Twenty-five patients underwent PT (16 men, 9 women; mean age 44.2 years; range 21 to 53 years), and 26 patients underwent ST (18 men, 8 women; mean age 46.1 years; range 24 to 56 years). Results: There were no significant differences with regard to complications, vvhich were of slight severity. Durations for the preparation of PT and CT were 19.0+6.3 and 28.7+6.9 min (p<0.01); durations of procedures were 6.8+3.2 and 18.3±4.1 min (p<0.01), and overall durations were 32.1 ±11.2 and 56.2±10.7 min (p<0.001), respectively. Conclusion: Our results suggest that percutaneous tracheostomy is a simple and safe bedside procedure, requiring shorter time and fewer health personnel when compared to surgical tracheostomy.
