Elektif sezaryen ameliyatlarında epidural %0.5 Bupivakain ve %0.5 Ropivakain kullanımlarının karşılaştırılması

Özet
Elektif sezaryen ameliyatı geçirecek olgularda epidural yoldan ve tek doz analjezi uygulamalarında bupivakain ve ropivakainin etkileri karşılaştırıldı. Hastalar ve Yöntemler: Çalışmaya ASA l-ll grubuna giren, yaşları 20-40 arasında değişen 40 olgu alındı. Olguların SAB, DAB, OAB, KAH ve SpO2 değerleri kaydedildi. Sol yan dekübitis pozisyonda, L4.5 intervertébral aralıktan girilerek epidural kateter yoluyla rastgele seçilen 20 olguya %0.5'lik bupivakain, 20 olguya %0.5'lik ropivakainle hazırlanmış 20 ml'lik solüsyon enjekte edildi. Bulgular: Çeşitli dermatomlarda sensoryal analjezinin başlama ve en üst seviyeye ulaşma zamanı ile motor blok geri dönüş süreleri ropivakain grubunda daha kısa; toplam analjezi süresi daha uzun; motor blok derecesi daha düşük bulundu; bu farklılıklar istatistiksel olarak ileri derecede anlamlı idi (p<0.01). Sensoryal analjezinin iki segment gerileme zamanı, ropivakain ile daha uzun olmak üzere, istatistiksel olarak çok ileri derecede anlamlı bulundu (p<0.001). Her iki grupta da ilk 30 dakika içinde SAB, DAB, OAB değerlerinde düşme, KAH değerlerinde artma gözlendi. Bupivakain grubunda SAB, DAB, OAB'daki düşüşler ve KAH'daki artma; ropivakain grubundaki DAB ve OAB'daki düşüşler anlamlı derecedeydi (p<0.05). Diğer parametreler yönünden, gruplar arasında fark belirlenmedi. Sonuç: Sezaryen ameliyatlarında, ropivakainin bupivakaine tercih edilebileceği kanısına varıldı.
Abstract
We compared the epidural administration of a single dose bupivacaine or ropivacaine in parturients undergoing elective cesarean section. Patients and Methods: The study included 40 term parturients (ASA l-ll) of ages ranging from 20 to 40 years. The values of SBP, DBP, MAP, HR, and SpO2 were recorded. The epidural catheter was inserted at the level of L4.5 in the left lateral decubitus position. The patients were randomly assigned to 0.5% bupivacaine (n=20) or ropivacaine (n=20) in a 20 ml solution. Results: The recorded durations for the beginning of sensorial analgesia at various dermatomes, for sensorial analgesia to reach the maximum upper level, and for motor block to fade were shorter, and the degree of motor block was less with ropivacaine, whereas the total time for analgesia was longer, all being statistically significant (p<0.01). The time to two-segment regression for sensorial analgesia was highly significantly longer with ropivacaine (p<0.001). Over the first 30 minutes, both groups demonstrated decreases in SBP, DBP, and MAP values, and increases in HR values. Decreases in SBP, DBP, MAP and increase in HR in the bupivacaine group, and decreases in DBP and MAP in the ropivacaine group were significant (p<0.05). No differences were found between the two groups with respect to other parameters. Conclusion: The epidural use of ropivacaine may be preferred over bupivacaine in cesarean section operations.
