23 gauge transkonjonktival sütürsüz pars plana vitrektomi uygulanan gözlerde göz içi tamponadların göz içi basıncına etkisi

Özet
Amaç: Pars plana vitrektomide (PPV) kullanılan göz içi tamponadların göz içi basıncı (GİB) üzerine olan etkisini karşılaştırmak. Gereç ve Yöntemler: Beyoğlu Göz Eğitim ve Araştırma Hastanesi’nde çeşitli endikasyonlarla Şubat 2008 ile Ocak 2009 tarihleri arasında 23 gauge (G) PPV ile beraber göz içi tamponad uygulanan 40 hastanın 40 gözü retrospektif olarak incelendi. Grup 1 (n=17) endotamponad olarak silikon (1000 cst) yağı uygulanan gözler, Grup 2 (n=23) endotamponad olarak gaz (SF6 veya C3F8) uygulanan gözler alındı. İki grup ameliyat öncesi ve sonrası 1. gün, 1. hafta, 1. ay, 3. ay ve 6. aylarda GİB değişimi açısından karşılaştırıldı. Bulgular: Grup1 hastalarının 8’i (%47) kadın 9’u (%53) erkek olup hastaların yaş ortalaması 48 yıl (8-68 yıl) idi. Ortalama takip süresi 6.4 ay (4-8 ay) idi. Grup 2 hastalarının 11’i (%48) kadın 12’i (%52) erkek olup hastaların yaş ortalaması 57.8 yıl (8-82 yıl) idi. Ortalama takip süresi 7.4 ay (6-9 ay) idi. Grup 1 hastalarının 8’inde (%47) medikal tedavi ile kontrol altına alınan glokom görülürken 3’ünde (%17) geçici GİB yükselmesi görüldü ve 1(%5.8) hastada geçici hipotoni gözlendi. Grup 2 hastalarından 2 (%8.6) gözde medikal tedavi ile kontrol altına alınan glokom görülürken, 6 (%26.0) gözde geçici GİB yükselmesi ve 3 (%13) gözde de geçici hipotoni görüldü. Takip süresi içerisinde glokom ilacı gerektiren olgu sayısı Grup 1’de anlamlı derecede daha yüksekti (p=0.01). Geçici GİB yükselmeleri ve hipotoni açısından gruplar arasında anlamlı fark bulunmadı ( p1=0.6, p2=1.0). Sonuç: Vitrektomide endotanponad olarak slikon uygulanan gözlerde glokom görülme oranı gaz uygulanan gözlere göre anlamlı olarak daha fazla iken gaz uygulanan gözlerde daha çok geçici GİB yükselmesi görülür.
Abstract
Purpose: To asses the effect of endotamponades on intraocular pressure (IOP) after 23 gauge pars plana vitrectomy (23 gauge (G) PPV). Materials and Methods: We retrospectively reviewed the medical records of 40 eyes/40 consecutive subjects who underwent 23 G PPV in Beyoğlu Eye Training and Research Hospital between February 2008 and January 2009. Silicon oil (1000 cst) was used in 17 eyes (Group 1), while gas endotamponade (C3F8 and SF6) was used in 23 eyes (Group 2) at the completion of surgery. The change in IOP was assessed and compared in both groups at baseline and 1 day, 1 week, 1 month, 3 months and 6 months after the surgery. Results: In Group 1 the mean age was 48±15.8 years (8 female/9 male) and the mean follow-up was 6.4 (4-8) months. In Group 2 the mean age was 57±16.5 years (11 female/12 male) and the mean follow up was 7.4 (6-9) months. During the follow-up period in Group 1, 8 eyes (47%) had elevation in IOP controlled by medical therapy, 3 eyes (17%) had transient elevation in IOP and 1 eye (5.8%) had hypotonia while in Group 2 the rates were 2 eyes (8.6%), 6 eyes (26.0%) and 3 eyes (13%) respectively. The rate of elevated IOP controlled by glaucoma medication was significantly higher among cases in Group 1 (p=0.01). No significant difference was present in transient elevation in IOP (p=0.60) and hypotonia (p=1.00) between the groups. Conclusion: Elevation in IOP after 23 G PPV was more frequently seen in eyes with SiO while in eyes with gas endotamponade transient IOP elevation was more common.
