Özet

Bu yazıda, infiltratif duktal tip meme kanseri tanısı olan olguda, görme kaybı yapan seröz retina dekolmanının (SRD) etiyolojik ayırıcı tanısını yapmak amaçlanmıştır. Kırk dokuz yaşında, kadın hasta sol gözde bir haftadır olan görme kaybı şikayetiyle başvurdu. Sistemik sorgulamada, 5 yıl önce meme kanseri nedeniyle opere edildiği, kemoradyoterapi aldığı öğrenildi. Hasta, hipofiz mikroadenomu nedeniyle 10 mg deksametazon kullanıyordu ve uzun süredir kullandığı steroid tedavisine bağlı Cushing Sendromu şüphesiyle izlenmekteydi. Hastanın ilk başvuru sırasında, görme keskinliği (GK) sağda 1.0, solda 0.7, fundus muayenesinde, solda maküler eksudalar, flöresein anjiografide bilateral yaygın pencere defektleri ve OKT’de solda dış retinal katmanlarda ödem mevcut idi. Bu bulgularla ön tanı olarak santral seröz korioretinopati (SSKR) düşünülen ve takip önerilen hasta, 15 gün sonra ani görme kaybı ile döndü; muayenede GK; sağda 0.1, solda 1.5 mps ve bilateral seröz retina dekolmanı mevcut idi. Bu aşamada kullanmakta olduğu sistemik steroid kesildi. Bir ay sonra, GK sağda 0.7; solda 0.2, 7 ay sonraki kontrolünde ise GK sağda 1.0, solda 0.3 olup seröz retina dekolmanı tamamen çekilmiş idi. Hastadaki SRD’nin ve görme kaybının kendiliğinden düzelmesi metastatik tümörden ziyade SSKR’yi düşündürmüştür.

Abstract

We aimed to describe serous retinal detachment (SRD) in a case with infiltrative ductal type breast cancer. A 49 yearold woman presented with blurry vision on her left eye for the last week. On her medical history, she had a surgery and chemotherapy 5 years ago because of breast cancer. She was on 10 mg dexamethazone treatment for hypophysis mikroadenoma and she was on follow up for suspected Cushing Syndrome. Her visual acuity (VA) was 1.0 on the right eye and 0.7 on the left with macular exudates in the right eye, bilateral window defects in fluorescein anjiography, and edema of outer retinal layers in OCT. Central serous chorioretinopathy (CSR) was the first in differential diagnosis and she was determined to be followed without treatment. She came back with loss of vision on both eye 15 days later to our clinic. VA was 0.1 on the right eye and finger counting from 1,.5 meter on the left eye. Systemic steroid treatment was stopped then. VA increased to 0.7 in the right and 0,2 in the left eye spontaneously within a month. Her final VA was 1.0 on the right eye and 0.3 on the left eye with total regression of serous retinal detachment. In this particular case, CSR was the diagnosis because of spontaneous resolution of SRD and increase in visual acuity after cessation of steroid.
