Özet

Amaç: Katarakt cerrahisi sırasında veya sonrasında vitreusa düşen intraoküler lenslerin pars plana vitrektomi (PPV) yaklaşımı ile tedavileri, komplikasyonları ve sonuçlarını değerlendirmek. Yöntem: 1996-1999 yılları arasında kliniğimizde tedavi edilen ardışık 15 intraoküler lens (İOL) dislokasyon olgusu çalışma kapsamına alındı. Dislokasyon sebebi 13 olguda (%86.6) arka kapsül yırtılması ve 2 olguda (%13.3) erken postoperatif travma idi. Olguların 10'unda İOL repozisyonu (1 göz endokapsüler, 7 göz ön kapsül önüne, 2 göz tek bacak ön kapsül önüne diğer bacak skleral sütürasyon), 5'in de ise İOL değişimi (skleral fiksasyonlu İOL) gerçekleştirildi. Bulgular: Ortalama 7.5 aylık takip sonunda tüm olguların 11'inde (%72) 5/10 ve üzerinde görme elde edildi. Buna karşın 5/10 ve üzeri görme, İOL repozisyonu grubunda %90 (9/10), İOL değişimi gurubunda ise %40 (2/5) oranında saptandı. Postoperatif 4 olguda (%26.6) kistoid makula ödemi (KMÖ), 1 olguda (%6.6) retina dekolmanı (RD), 2 olguda (%13.3) kornea ödemi ve 2 olguda (%13.3) İOL desantralizasyonu gelişti. KMÖ ile RD gelişen gözlerin İOL değişimi grubunda yer aldığı ve 5/10 altı görmenin nedeni olduğu gözlendi. Sonuç: Her ne kadar iki yöntemle de iyi sonuçlar elde edilse de İOL değişimi uygulanan grupta görmeyi etkileyen komplikasyonlar dikkate alındığında disloke İOL'lerin olabildiğince mevcut kapsülden yararlanılarak repozisyonunun tercih edilmesi gerektiği kanısındayız.

Abstract

Purpose: To evaluate the results and complications of dropped intraocular lenses (IOL) during cataract surgery or follow-up period treated with pars plana vitrectomy. Methods: Fifteen eyes with dislocated IOL treated in our clinic between 1996-1999 were studied. The ethiology of the dislocated IOL were posterior capsul perforation in 13 eyes (86.6%) and early postoperative trauma in 2 eyes (13.3%). In 10 eyes IOL reposition (1 eye endocapsular, 7 eyes sulcus implantation and 2 eyes one haptic anterior capsular the other haptic scleral fixation) and in 5 cases IOL exchange was performed. Results: After a mean follow up period of 7.5 month, a total of 11 eyes (72%) had equal to or higher than 5/10 snellen visual aquity. But the same value of visual acuity improvement was 90% (9/10) in IOL reposition group and 40%(2/5) in IOL exchange group. Postoperative complications were cystoid macular edema (CME) in 4 eyes (26.6%), retinal detachment (RD) in 1 eyes (6.6%), corneal edema in 2 eyes (13.3%) and IOL desantralisation in 2 eyes (13.3%). CME and retinal detachment complications were all in IOL exchange group, and they were the cause of bad visual prognosis (<5/10). Conclusion: Apart from the good anatomic results in both group, when we consider worse visual result in the IOL exchange group, we suggest to preserve the remnant of lens capsule as much as possible to be able to replace IOL instead of exchange with scleral fixation IOL.
