Karpal tünel sendromlu hastalarda el-el bilek istirahat splinti ve lokal kortikosteroid enjeksiyonunun etkinliklerinin klinik, elektrofizyolojik ve ultrasonografik yöntemlerle değerlendirilmesi

Özet

Karpal tünel sendromu (KTS) tedavisinde kortikosteroid enjeksiyonu ve el-el bilek istirahat splintinin etkinliklerini değerlendirmek amacıyla bu çalışma planlandı. Ellerde uyuşma yakınması ile başvuran, klinik ve elektrofizyolojik değerlendirme ile KTS tanısı koyulan 31 hastanın 41 eli çalışmaya alındı. Çalışmada diabetes mellituslu hastalara el-el bilek istirahat splinti uygulandı, diğer hastalar randomize olarak iki gruba ayrıldı. Birinci gruba lokal kortikosteroid enjeksiyonu, ikinci gruba ise el-el bilek istirahat splinti uygulandı. Tedavi öncesinde ve tedavi baþlangıcından üç hafta sonra klinik olarak Phalen, ters Phalen, Tinel testleri, duyu muayenesi, ağrı ve parestezi için vizüel analog skala (VAS) ve ultrasonografide karpal tünel girişinde median sinir kesit alanı ve ekojenitesi, elektrofizyolojik olarak ise median sinir distal motor ve duyu latansı, birleşik kas aksiyon potansiyeli ve duyu amplitüdleri ile duyusal sinir iletim hızı değerlendirildi. Tedavi sonrası iki grubun karşılaştırılmasında; splint grubunda VASparestezi'de ve median sinir distal motor latansındaki azalma istatistiksel olarak daha anlamlı iken, enjeksiyon grubunda ise median sinir ekojenitesindeki düzelme daha belirgindi. Diğer parametrelerde iki grup arasında istatistiksel olarak anlamlı farklılık yoktu. Klinik ve elektrofizyolojik parametrelerle birlikte ultrasonografide median sinir kesit alanında anlamlı azalma gözlendi. Sonuç olarak her iki yöntem de KTS tedavisinde kısa dönemde etkili bulundu. Ayrıca ultrasonografinin de median sinirin görüntülenmesinde ve tedavi etkinliğinin değerlendirilmesinde kullanılabilecek bir yöntem olduğu gözlendi.

Abstract

This study was planned to evaluate the effect of local corticosteroid injection and wrist splinting in carpal tunnel syndrome (CTS). 31 patients (41 hands) who had paresthesia in the hand and diagnosed clinically and electrophysyologically as CTS were included in the study. Patients who had diabetes mellitus were treated with wrist splinting, the others were randomly divided into two groups. First group was treated with corticosteroid injection and the second with wrist splinting. Phalen, reverse Phalen and Tinel signs, sensorial examination, visual analogue scale (VAS) for pain and paresthesia, median nerve cross-sectional area at the carpal tunnel inlet, median nerve echogenity, median nerve distal motor and sensory latencies, compound muscle action potential and sensory amplitudes and sensory nerve conduction velocity were evaluated before treatment and three weeks after the treatment. At the end of the study; significant reduction was seen in VASparesthesia and median nerve distal motor latency in the wrist splinting group compared to the injection group, but improvement in median nerve echogenity was higher in the injection group. There was difference in the other variables between two groups. A significant decrease was seen in median nerve cross-sectional area in ultrasonography. In conclusion, both treatment modalities were found as effective in CTS in short-term followup. Besides, ultrasonography was found to be a useful tool for screening of median nerve and evaluation of the treatment efficacy.
