Özet
Amaç: Yaş tip yaşa bağlı makula dejenerasyonlu (YBMD) hastalarının tedavisinde intravitreal pegaptanib sodyum enjeksiyonunun etkinliğini araştırmak. Gereç ve Yöntem: Dokuz Eylül Üniversitesi Göz Hastalıkları Retina Birimi’nde yaş tip YBMD tanılı, intravitreal pegaptanib sodyum enjeksiyonu ile tedavi edilen 52 hastanın 52 gözü retrospektif olarak incelendi. Tüm anjiografik lezyon tipleri çalışmaya dahil edildi. Pegaptanib sodyum enjeksiyon sayısı, takip süreleri, aldığı ek tedaviler, tedavi öncesi ve son kontrollerindeki görme keskinlikleri değerlendirildi. Bulgular: Hastaların 23’ü kadın, 29’u erkekti. Ortalama yaş 71.4±10.5 olarak bulundu. Lezyonların %46’sında(24/52) baskın klasik tip lezyon, %29’unda(15/52) gizli tip lezyon ve %25’inde(13/52) pigment epitel dekolmanı (PED) mevcuttu. Ortalama takip süresi 6.2±3.1ay ve ortalama pegaptanib enjeksiyonu sayısı 2.0 (1-5) olarak bulundu. Takip süresinde ilaveten 10 hastaya 1 kez intravitreal bevacizumab enjeksiyonu, 15 hastaya 1 kez fotodinamik tedavi (FDT), 1 hastaya 2 kez FDT, 7 hastaya 1 kez intravitreal triamsinolon enjeksiyonu da uygulanmıştır. Hastaların tedavi başlangıcında ortalama görmeleri 0.97±0.3 logMAR ünit iken, son görmeleri 0.96±0.3 logMAR ünit olarak saptandı. Tedavi başlangıcı ve takip süresi sonundaki görme düzeyleri arasında istatistiksel olarak anlamlı fark saptanmadı (p>0.05). Hastaların hiçbirinde intravitreal pegaptanib sodyum enjeksiyonuna ait ciddi komplikasyon gelişmedi. Sonuç: Çalışmamızda, takip süresinde hastaların görme keskinlikleri korunmuştur. İntravitreal pegaptanib sodyum tedavisi yaş tip YBMD hastalarında güvenilir bir alternatiftir.
Abstract
Purpose: To evaluate the efficacy of intravitreal pegaptanib sodium injection in the treatment of wet type age-related macular degeneration (AMD). Materials and Methods: 52 eyes from 52 patients treated for wet type AMD were evaluated retrospectively at the Retina unit of Dokuz Eylul University Ophthalmology Department. All subtypes of wet type AMD were included. Pre and post injection visual acuities, number of pegaptanib injections, additional treatment modalities and follow up period were assessed. Results: Twenty three patients were female and 29 were male. Mean age was 71.4±10.5 years. Fourtysix% (24/52) of the lesions were predominantly classic, 28.8% (15/52) occult and 25% (13/52) pigment epithelial detachment. The mean follow up was 6.2±3.1 months. The average number of pegaptanib injections was 2.0 (1-5). During the follow up, intravitreal bevacizumab was adminstrated in 10 patients, intravitreal triamcinolon injection was given to 7 patients and a single photodynamic treatment (PDT) was performed in 15 patients and two PDT sessions were given to one patient. Mean visual acuity was 0.97±0.3 logMAR unit at initial examination and 0.96±0.3 logMAR unit at the end of follow up. There was no statistically significant difference between the visual acuities (p>0.05). No serious complications were observed after the pegaptanib injections. Conclusion: Intravitreal injection of pegaptanib sodium resulted in visual acuity stabilisation in our group of patients. Intravitreal injection of pegaptanib Na is a safe alternative in wet-type AMD.
