Özet

Akut postoperatif endoftalmi nedeni ile başvuran 80 yaşındaki erkek hastaya pars plana vitrektomi ve intravitreal vankomisin (1 mg/0.1 ml) ve seftazidim (2.25 mg/0.1 ml) enjeksiyonu yapıldı. Cerrahi ve antibiyotiklere rağmen iyileşme olmadı. Vitreus örneğinin kültüründen üreyen Pseudomonas aeruginosa imipeneme duyarlı olduğu için sistemik ve intravitreal imipenem uygulandı. Tedaviden iki gün sonra vitritis ve ön kamara inflamasyonu azaldı. Bir ay sonra görme keskinliği artarak 4 metreden parmak sayma düzeyine ulaştı. Endoftalmi katarakt ameliyatının en ciddi komplikasyonları arasındadır. İmipenem Pseudomonas aeruginosadan kaynaklanan endoftalminin tedavisinde etkilidir. Mikroorganizmaların duyarlı olduğu uygun antibiyotikle tedavi ile enflamasyon başarı ile kontrol altına alınabilmekle birlikte pseudomonasın etken olduğu endoftalmili hastalarda görme prognozu kötüdür.

Abstract

A 80 year-old-male presented with acute postoperative endophthalmitis. The patient underwent pars plana vitrectomy and intraocular antibiotic injection of vancomycin (1 mg/0.1 ml) and ceftazidime (2.25 mg/0.1 ml). Despite surgery and antibiotics, there was no improvement. Pseudomonas aeruginosa was isolated from vitreous. Since the isolated organism was sensitive to imipenem, intravenous and intravitreal imipenem injection was performed. Two days after the treatment vitritis and anterior chamber inflammation decreased. Over the next month visual acuity gradually improved to counting finger at four meters. Endophthalmitis is among the most serious complications of cataract surgery. Imipenem is effective in the management of endophthalmitis caused by Pseudomonas aeruginosa. Although inflammation can be controlled with prompt antibiotic treatment to which the organisms were sensitive visual prognosis continues to be poor in pseudomonas endophthalmitis patients.
