Prematüre retinopatisinde takip ve tedavi sonuçlarımız

Özet
Amaç: Prematüre retinopatisi (PR) sıklığını tanımlamak ve argon lazer fotokoagulasyon (ALF) ve intravitreal bevacizumab (İVB) enjeksiyonu ile tedavi sonuçlarını bildirmek. Gereç ve Yöntem: Haziran 2010 ile Mayıs 2011 tarihleri arasında muayene edilen prematüre bebeklere ait kayıtlar geriye dönük olarak incelendi. Doğum ağırlığı, gestasyonel yaş, retinopati evresi ve zonu araştırıldı. Eşik öncesi hastalık saptanan bebeklere indirek ALF±İVB tedavisi uygulandı. Bulgular: İkiyüzseksenbeş prematüre bebeğe ait kayıtlar incelendi. Ortalama gestasyonel yaş 29,9±3,1 hafta ve ortalama doğum ağırlığı 1443±518 gram idi. Prematüre retinopati 180 olguda saptandı. Otuz hastanın 60 gözü ALF ile tedavi edildi. Beş hastanın sekiz gözü ek İVB ile tedavi edildi. Düşük doğum ağırlığı ve düşük doğum haftası olan bebeklerde PR evresi daha ileri saptandı (p<0.001). 30 hastanın 28’inde tedavi sonrasında PR geriledi. Önceden eşik hastalık tanısı konan 2 hastanın 2 gözünde ise retina dekolmanı gelişti. Sonuç: Eşik öncesi hastalıkta ALF tedavisi sonuçları başarılı ve komplikasyonlar daha düşüktür. İVB ile adjuvan tedavi, özellikle ortam bulanıklığı olan gözlerde ve ALF tedavisine yanıtın düşük olduğu gözlerde oldukça etkili ve ümit vaat edici gözükmektedir.
Abstract
Purpose: To determine the frequency of retinopathy of prematurity (ROP) and the results of the treatment with argon laser photocoagulation (ALP) and intravitreal bevacizumab (IVB) injection. Material and Methods: The records of the premature babies were retrospectively evaluated between June 2010 and May 2011. Birth weight, gestational age, retinopathy grade and zone were investigated. Indirect ALP ± IVB were performed when prethreshold ROP was detected. Results: The records of 285 premature babies were evaluated. Of them, 150 were male, and 135 were female. Mean gestational age was 29.9±3.1 weeks and mean birth weight was 1443±518 g. ROP was detected in 180 patients. Sixty eyes of 30 patients were treated with ALP. Eight eyes of 5 patients were treated with adjuvant IVB. In patients with lower gestational age and lower birth weight, the stage of the ROP was more advanced (p<0.001). ROP was regressed in 28 of 30 patients after treatment. Retinal detachment developed in 2 eyes of 2 patients, who had previously diagnosed as threshold disease. Conclusion: the results of ALP treatment at the prethreshold disease were successful, and the complications were lower. Adjuvant treatment with IVB is very effective and promising, especially in eyes with hazy media and/or refractory to the ALP treatment.
