İdiopatik Koroidal Neovasküler Membranlı Çocuk Olguda Tek Doz İntravitreal Aflibercept Tedavisi

Özet
Ondört yaşındaki kız olguda ortaya çıkan ve tek doz intravitreal aflibercept ile tedavi edilen idiopatik koroidal neovasküler membran (KNVM) olgusunun sunulması amaçlanmıştır.Ondört yaşındaki kız hasta kliniğimize sol gözde görme azalması şikayeti ile başvurdu. Sol gözde görme keskinliği 0.2 düzeyindeydi ve fundus muayenesinde subfoveal koroidal neovasküler membran ile uyumlu görünüm mevcuttu. Hastanın çekilen optik koherens tomografi ve fluoresein anjiografisinde klasik KNVM saptandı . Eşlik eden sistemik ve oküler patolojisi olmayan hastaya idiopatik KNVM tanısı konuldu ve tek doz intravitreal aflibercept (Eylea®) 2mg/0.05 ml uygulandı. Enjeksiyon uygulanmasının ardından birinci ayda sol gözde görme düzeyi 0.6 seviyesine yükseldi. Hastanın bir yıllık takibinin sonunda görme düzeyi 0.8 seviyesinde sabit kaldı ve nüks izlenmedi.Sonuç: Çocuklarda nadir görülen bir hastalık olan idiopatik KNVM'de tek doz intravitreal aflibercept enjeksiyonu ile herhangi bir yan etki olmaksızın anatomik ve fonksiyonel olarak belirgin düzelme izlenmiş ve 1 yıllık izlemde nüks saptanmamıştır.
Abstract
To report a 14 year old girl with idiopathic choroidal neovascular membrane (CNVM) treated with single dose intraviteral aflibercept injection.A 14 year old girl who presented with loss of vision in the left eye. Visual acuity was 0.2 and subfoveal choroidal neovascular membrane was detected in the left eye. As a result of angiographic and optic coherens tomographic evaluation, classic CNVM was found. No associated ocular or systemic diseases was detected and the lesion was evaluated as idiopathic CNVM. Single dose of intravitreal aflibercept (Eylea®) 2 mg/0.05 ml was injected into the left eye. At a month follow-up visit, visual acuity improved to 0,6. At the end of a year visual acuity was 0.8 in the left eye and no recurrence was noted during follow-up.Conclusion: Idiopathic CNVM is very rare in children and treatment with single dose intravitreal aflibercept can result anatomical and functional success during one year follow-up without side effects and recurrence.
