Özet

Herediter retina distrofileri (HRD) klinik ve genetik açıdan çe- şitlilik gösteren retinanın dejeneratif hastalıklar grubudur. Bu hastalıklar gece görme bozukluğu, renk görme bozukluğu, görme alanı kaybı ve total körlükle sonuçlanabilir. Bu hastalık grubun- da gelişimsel ve fonksiyonel aşamada rol alan 120 den fazla gen mutasyonunun sorumlu olabileceği saptanmıştır. Son yıllarda meydana gelen gelişmeler genetik patogenezin daha iyi anlaşıl- masına ve gen replasman tedavilerinin uygulanabilmesine yol açmıştır. Anatomik ve immunolojik özelliklerinden dolayı göz gen tedavi- lerinin uygulanabilmesi için uygun bir organdır. Son gelişmeler ışığında vektör aracılı gen replasman tedavileri oldukça yol ka- tetmiştir. İlk klinik çalışmalardan alınan sonuçlar oldukça ilgi uyandırmış ve insanlarda intraokuler viral vektörlerin güvenli ve etkili olduğunu göstermiştir. Kök hücreler farklılaşmamış, kendi kendini yenileyebilme ve vücuttaki diğer matür hücrelere dönüşebilme yeteneğine sahip hücrelerdir. Bu hücrelerin yüksek proliferasyon yeteneği vardır ve bir kök hücre kaynağından sayısız matür hücre gelişebilir. Bu nedenle son yıllarda değişik patolojiler için hücre replasman te- davisi çalışılmaktadır. Bu tedavi ile hastalıklı retinadaki hasarlı retina hücrelerinin kök hücre kaynağından farklılaşan yeni reti- na hücreleri ile yer değiştirmesi amaçlanmaktadır. Kök hücreler ayrıca immunoregülasyon, nöronlarda anti-apopitozis, nörotrofin sekresyonu gibi pek çok farklı fonksiyon da gösterir. Son yıllar- da deneysel uygulamalarda meydana gelen gelişmeler ışığında HRD‘de kök hücre kullanımı ile ilgili faz I/II klinik çalışmalar yapılmış ve cesaret verici sonuçlar alınmıştır. Bu derlemede HRD’lerinde gen tedavisi ve kök hücre konusundaki son gelişme- ler ve klinik çalışmaların sonuçları özetlenmiştir.

Abstract

Hereditery retinal dystrophies (HRDs) are degenerative diseases of the retina which have marked clinical and genetic heterogene- ity. Common presentations among these disorders include night or colour blindness, tunnel vision and subsequent progression to complete blindness. The known causative disease genes have a variety of developmental and functional roles with mutations in more than 120 genes. Signifi cant advancements have been made in understanding the genetic pathogenesis of ocular diseases, and gene replace- ment has been implicated as an potentially efficacious therapy. Because of its favorable anatomical and immunological charac- teristics, the eye has been at the forefront of translational gene therapy. Recent improvements have been made in the safety and specifi city of vector-based ocular gene transfer methods. The re- sults from the first clinical trials have generated great interest and have demonstrated the safety and efficacy of intraocular ad- ministrations of viral vectors in humans. Stem cells are undiff erentiated cells which have the ability to self-renew and diff erentiate into mature cells. They are highly proliferative, implying that an unlimited number of mature cells can be generated from a given stem cell source. On this basis, cell replacement therapy has been evaluated in recent years as an alternative for various pathologies. Cell replacement therapy hypothesizes that new retinal cells could be generated from stem cells so as to replace the damaged cells in the diseased retina. In addition, stem cells are able to perform multiple functions, such as immunoregulation, anti-apoptosis of neurons and neuro- trophin secreting. With recent progress in experimental stem cell applications, several phase I/II clinical trials were approved and we have the promising and encouraging results from these stem cell clinical trials. This review summarizes the clinical develop- ments of stem cells and retinal gene therapy in HRD.
