Fasyal paralizilerin cerrahi tedavisi ve sonuçları

Özet
Amaç: Fasyal paralizi nedeniyle cerrahi tedavi uygulanan hastaların cerrahi bulgu ve sonuçlan incelendi. Hastalar ve Yöntemler: Fasyal paralizi nedeniyle cerrahi tedavi uygulanan 77 hastanın (49 erkek, 29 kadın; ort. yaş 26; dağılım 1-72) hastane kayıtları retrospektif olarak değerlendirildi. Hastalar etyoloji, başvurma süreleri, cerrahi bulgu ve iyileşme düzeyleri bakımından karşılaştırıldı. Fasyal paralizi nedenleri 33 hastada Bell paralizisi, 30 hastada travma, sekiz, hastada iatrojenik, dört hastada kronik otit komplikasyonu, iki hastada doğuştan idi. Bulgular: Travma nedeniyle fasyal paralizi geçiren hastaların 21'ine fasyal sinirdeki ödem nedeniyle fasyal dekompresyon, dokuzuna sinirde keşi nedeniyle dekompresyon ve anastomoz uygulandı. Bell paralizili hastalara transmastoid dekompresyon uygulandı; 20 hastada mastoid, iki hastada timpanik, bir hastada stilomastoid, beş hastada timpanik ve mastoid segmentlerde ödem belirlendi. Cerrahi sonrasında 42 hasta izlenebildi; bunların yalnızca dördünde tedaviden yarar sağlanamadı. Travmatik fasyal paralizi nedeniyle cerrahi uygulanan 16 hastanın tümünde iyileşme gözlendi; Bell paralizili 17 hastada (%85) iyileşme, üçünde (%15) tam düzelme görüldü, iatrojenik fasyal paralizi olan hastaların tamamı, kronik otit nedeniyle fasyal paralizi geçiren hastaların ise ikisi iyileşti. Sonuç: Diğer nedenlerle oluşan fasyal paralizilerde tedavi yöntemleri konusunda görüş birliği olmasına rağmen, Bell paralizisinin patofizyolojisi ve tedavisi üzerine daha fazla bilimsel çalışmaya gerek vardır.
Abstract
Objectives: We evaluated surgical findings and the results of surgical therapy in patients with facial paralysis. Patients and Methods: Hospital records of 77 patients (49 males, 29 females; mean age 26 years; range 1 to 72 years) who were treated surgically were reviewed. The patients were compared with regard to etiology of paralysis, the length of time for treatment from onset, surgical results, and recovery levels. Facial paralysis resulted from Bell's palsy (n=33), trauma (n=30), complications of chronic otitis media (n=4), and iatrogenic (n=8) and congenital (n=2) causes. Results: Facial paralysis due to trauma was treated with facial nerve decompression with (n=9) or without (n=21) anastomosis because of edema of the nerve. We performed transmastoid decompression for Bell's palsy and observed edematous nerve in the mastoid (n=20), tympanic (n=2), stylomastoid (n=1) and tympanomastoid segments (n=5). Follow-up evaluations included 42 patients, four of whom did not benefit from surgical treatment. Ali the patients (n=16) vvith traumatic paralysis achieved recovery of facial functions. Three patients (15%) with Bell's palsy had complete recovery, while 17 patients (85%) had improvement in facial functions. AH the patients with iatrogenic paralysis and two patients with complications of chronic otitis media had complete recovery. Conclusion: Although there is agreement on the treatment of facial paralysis due to other causes, further studies are required to clarify the pathophysiology and treatment of Bell's palsy.
