Özet

Osleoartritin heterojen yapısı içinde seyrini ve sonuçlarını değerlendirebilmek için anlamlı ölçeklerin olmayışı bu konudaki gelişmeleri engellemektedir. Osteoartritte ağrı ve radyografik bulguların her zaman ilişkili olmadığı gözlemlenmiştir. Radyografik bulgular ile disabilite ilişkisi ise yeterince araştırılmamıştır. Bu çalışmada OA'lı bir hasta grubunda radyografik bulgular ile disabilite ve hastalık şiddetinin (Lequesne severity indeks) objektif yöntemlerle ölçümleri yapılarak bunlar arasındaki ilişkiler araştırılmıştır. Amerikan Romatizma Cemiyeti (ACR) kriterlerine göre tanısı konmuş yaş ortalamaları 57±8,6 olan 71 OA'li hasta (63 kadın,8 erkek) çalışmaya alındı. Hastalar klinik paternlere (monoartiküler, diz-el ve yaygın) ayrıldı. Hastalarda klinik paternler arasında yaş, radyografik indeks, hastalık süresi, ve disabilite yönünden fark bulunmadı (p 0.05). Ellerde ve dizlerde radyografik indeksler arasında anlamlı bir ilişki yoktu. Genel disabilite ise en çok yaş ve LŞİ ile ilişkili bulundu (p<0.01). Sonuç olarak OA'li hastaların takibinde LŞİ ve dizde medial eklem aralığının ölçülmesi, Health Assesment Questionnaire (HAQ) kullanılması değerli bir yöntem olarak görülmektedir.

Abstract

To evaluate the progress and outcomes of osteoarthritis (OA) in its heterogenity structure is important for the individual and the society. The absence of valuable scales is an handicap for the development in this area. It was observed that in osteoarthritis patients pain and radiological findings are not always correlated. The relationship between the radiological findings and disability is not dedicated before in detail. In this study in a series of osteoarthritis patients, disability and disease severity were measured by objective methods and the relationship between them was investigated. 71 patients (63 female and 8 male, the mean age was 57±8,6) with osteoarthritis were divided into one of three clinical patterns whether they were diagnosed as monoarticular, knee-hand or polyarticular osteoarthritis, according to American College of Rheumatism (ACR). There was not a significant difference between the clinical patterns in respect of radiographic index, disease duration and disability (p>0.05). Narrowing of the knee joint space and the radiographic index, was found to be well correlated with the age. Lequesne severity index (LSI) was positively related with the lower extremity disability index and narrowing of the joint space (p<0.001). The radiographic index did not differ between the groups. The relationship between radiographic index of hands and knees were not related. General disability was found to be related with LSI and age (p<0.001). In conclusion, LSI, measuring of the medial joint space in knees and HAQ (Health Assesment Questionnaire) seemed to be valuable methods in following up the OA patients .
