Özet
1989'da tanımlanmasından sonra o yıllardaki posttransfüzyonel hepatitlerin %90'ından sorumlu tutulan hepatit C virüsü (HCV), infeksiyonun %80'e varan oranlarda kronikleşmesi, siroz ve hepatoselüler karsinoma etyolojisindeki rolü, sağlık personeli için risk oluşturması, non-parenteral buluşların giderek artması, virüse karşı etkin hiperimmunglobulin ve aşının henüz üretilememiş olması gibi nedenlerle ülkemizde ve dünyada giderek önemi artan bir sağlık sorunudur. Özellikle Genel Cerrahi kliniklerinde çalışan sağlık personelinin bu açıdan karşı karşıya bulundukları tehlikenin boyutunu saptamak amacıyla Genel Cerrahi hasta popülasyonunda anti-HCV prevalansı saptanmış, ayrıca hastalar klinik ve epidemiyolojik Özellikleri yönünden incelenmiş ve hastanelerde ve toplumda alınması gereken önlemler konusunda önerilerde bulunulmuştur. Çalışmaya 1004 hasla alınmış, hastaların %2.6'sında (26 hasta) anti-HCV pozitif bulunmuştur. Bu oran, ülkemizde daha önce yapılmış prevalans çalışmalarında saptanan %1 ve altındaki değerlere göre oldukça yüksektir. Önceden kan transfüzyonu yapılanlarda ve erişkinlerde (30 yaş üzeri) anti-HCV seropozitiflik oranının anlamlı derecede yüksek olduğu saptanmıştır. Anti-HCV pozitif uç hastada aynı zamanda hepatit B yüzey antijeni de pozitif bulunmuştur. Cinsiyet, kaza-cerrahi operasyon veya diş müdahalesi geçirmiş olma, alkol kullanımıyla anti-HCV seropozitiflik oranı arasında anlamlı bir ilişki bulunamamıştır. HCV seropozitif hastalarda hepatitle ilgili olabilecek semptomların belirgin olmadığı saptanmıştır. Kan ve vücut sıvılarıyla bulaşabilin HCV'nin yüksek oranda mutasyona uğramasından dolayı tam bağışıklığın oluşamaması, kesin tedavisinin olmaması nedeniyle infekte hastaların getireceği yük korunma yollarının öğretilip uygulanmasından daha yüksek mali külfet getirmektedir. Bulaşıcı hastalıklar konusunda bilinen evrensel önlemlerin Önemsenerek sağlık çalışanları ve toplum tarafından titizlikle uygulaması, kontaminasyon konusunda daha az riskli tıbbi aletlerin ve cerrahi tekniklerin kullanılması, steril çalışma koşullarından ödün verilmemesi, hizmet içi eğitim ve topluma yönelik eğitimi ile kişilerin bilinçlendirilmesi korunmada etkili yöntemlerdir.
Abstract
After the definition of hepatitis C virus (HCV) in 1989, it was accepted to be responsible for 90% of posttransfusional hepatitis. Because 80% of the infection progresses to chronic form has a role in the etiology of cirrhosis an hepatocellular carcinoma, is an important risk for health care workers, non-parenteral contamination increase, effective hyperimmunoglobulin and vaccine against the virus has not been produced yet, HCV is a health problem with a rising importance in Turkey and in the world. In this study, to asses the dimension of risk in health care workers, the prevalence of anti-HCV was investigated among the general surgery patients. Besides, the clinical and epidemiological features of the patients were observed and the prevention methods to be followed in the hospitals and in the society were mentioned. Twenty-six (2.6%) of the 1004 patients studied were positive for anti-HCV. This ratio was higher than the ratios of 1% and below determined in the previous prevalence studies in Turkey. AntiHCV seropositivity was significantly higher in the adults (>30 years) and those who received blood transfusion before. Three patients, seropositive for anti-HCV, were also seropositive for HBsAg. There was no significant relation between anti-HCV seropositivity and prior accidents, surgery or dental procedures, alcohol use and sex. HCV seropositive patients had no evident symptoms that could be related to hepatitis. HCV, which could be transmitted by blood and body fluids, has a high mutation rate, so that, absolute immunity cannot occur and there isn't a radical treatment far HCV infection. Therefore, teaching and applying the prevention methods are less cost-effective than the management of infected patients. Application of universal prevention methods for infectious diseases by the health care workers and the society carefully, using safer medical instruments and surgical techniques which diminish the contamination risk, following sterilization methods fastidiously, education of health care workers and the people would be the effective prevention methods.
