Meme kanserli hastalarda metastaz durumunun diğer parametrelerle karşılaştırlması

Özet
Çalışmamızda meme kanserli hastaların histolojik tip, tümör boyutu, tutulan bölge, steroid reseptörleri ve metastaz durumlarını; metastaz durumu ile yaş grupları, tümör boyutu, tümör yerleşim yeri ve steroid reseptörleri arasında ilişki olup olmadığını inceledik. Ortaya çıkan bulgularla meme kanserinin tanısı ve metastaz durumunun değerlendirilmesine katkıda bulunmayı hedefledik Bizim çalışmamızda meme kanserinin en sık aksilla, kemik, akciğer ve karaciğere metastaz yaptığı tespit edildi. Aksiller lenf nodu metastazı oranı %69.6, kemik metastazı oranı %29.4 olup, kemik metastazı olanların %50'sinde tutulan bölgenin vertebra olduğu saptandı. Hastaların % 35.3'ünde akciğer tutulumu, % 8.8'inde karaciğer tutulumu olduğu, hastaların %25.5'inde teşhisten sonraki 36 ay içinde metastaz saptandığı belirlendi. Metastaz durumu diğer bulgularla karşılaştırıldığında histolojik tip olarak en fazla görülen infiltratif duktal karsinomun metastaz durumu ile anlamlı farklılık gösterdiği (P<0.05) ve steroid reseptörleri negatif olanlarda metastazların anlamlı derecede (P<0,05) daha fazla görüldüğü belirlendi. Sonuç olarak steroid reseptör durumunun bilinmesi prognoz ve tedavi açısından önemlidir.
Abstract
At breast cancer patients we studied histological type, size of the tumors, involved area, steroid receptors and presence of metastasis. Also we studied relationship between the presence of metastasis and age groups, tumor dimension, site of tumor, steroid receptors. With this findings we want to contribute the diagnose and evaluation of metastatic status of breast cancers. In our study we determined that the most frequent metastatic site of breast cancer are axilla, bone, lung and liver. The metastatic ratio of axiller lymph node is 69.6 percent and bone matastasis 29.4 percent. Fifty percent of metastatic bone lesions were at columna vertebralis. 35.3 percent of patients had lung, 8.8 percent had liver metastasis. After 36 months of diagnosis metastasis were found 25.5 percent of the patients. When we compared metastatic status and other findings we found that infiltrative ductal carcinoma is the most frequent hystological type of metastatic breast cancer (P<0.05) and metastasis of the steroid receptor negative tumros were significantly more frequent (P<0.05). As a result it is important to know steroid reseptor status of tumors for prognosis and treatment.
