Özet
Hemodiyaliz (HD), Son Dönem Böbrek Yetmezliği (SDBY)'nin kontrolünde kullanılan güvenilir ve etkin bir metoddur. Bununla birlikte SDBY nedeniyle HD 'e girme; fizyolojik, psikolojik ve sosyal düzeylerde etkileri olan, stresli bir yasam olayına dönüşebilmektedir. HD makinesi "yaşamı koruyucu" bir rol üstlenmekle birlikte; HD tedavisi, SDBY için bir "çözüm" değildir ve böbreklerin endokrin veya metabolik fonksiyonlarını yeniden kazanmasını sağlayamaz. Bu nedenle HD, hastanın yaşamının hayati bir parçası olur. Yaşamı sürdürebilmek için HD gibi invazif bir tedavinin zorunlu olması, bireyin yaşamının bütün yönlerini ve sağlığını etkileyebilmektedir. HD tedavisiyle ilgili teknolojideki hızlı gelişmelere rağmen, hastalar yeni teknik ilerlemelerle giderilemeyen, tedaviyle ilişkili birçok stressörle karşılaşmaktadırlar. SDBY nedeniyle HD'e giren hastalarda, önemli düzeyde stres yaşandığı bilinmesine karşın, hastalar tarafından bu durumla başetmede kullanılan mekanizmalar gözardı edilebilmektedir. Bu nedenle, hastaların kullandığı başetme yöntemlerine de dikkat edilmelidir.
Abstract
Hemodialysis (HD) is a safe and effective method of managing End-Stage Renal Disease (ESRD). However, ESRD necessitating treatment by HD is a stressful life event that impacts on physiological, psychological, and social levels. The HD machine is a "lifesaver", but not a "cure " for ESRD; nor does it replace the endocrine or metabolic Junctions of the kidney. HD becomes a vital part of the lives of HD patients. The need for such an invasive therapeutic measure to maintain life effects the health and virtually all other aspects of these individuals' lives. In spite of technical improvements patients are faced with numerous treatment-associated stressors that are not alleviated by the new advances in technology. While itiswellknown that ESRD, necessitatingHD, isa major stress among those afflicted, what is not well known are the mechanisms developed by individuals and groups for coping with this assault on their daily living. Little attention has been given to the active components of coping behavior utilized by patients. There is a need to evaluate not only the patient's perception of the stressfulness of the HD regimen, but also the types of coping patterns used to deal with the stresses of treatment.
