Hemodiyaliz Hastalarında Depresif Belirti Görülme Durumu

Özet
Hemodiyaliz tedavisinde sık karşılaşılan psikiyatrik bozukluklardan birisi depresyondur. Son dönem böbrek yetmezliği nedeniyle hemodiyaliz tedavisi alan hastalar ile dermatoloji poliklinik hastalarının depresif belirti görülme durumlarının karşılaştırılması amacıyla yapılan tanımlayıcı araştırmada, sosyodemografik özellikler, sigara ve alkol kullanma alışkanlıklarına yönelik sorular ve depresif belirti görülme durumlarını değerlendirmek amacıyla Beck Depresyon Ölçeği(BDÖ) kullanılmıştır. Katılımcıların yaş ortalaması 50,09±17,02'dir, %53,8'i(n=57) kadın, %67'si(n=71) evlidir. Diyaliz ünitesindeki katılımcıların(n=36) yaş ortalaması 62,06±14,03 dermatoloji polikliniğindeki katılımcıların(n=70) yaş ortalaması 43,94±15,10'dur. BDÖ puan ortalaması diyaliz ünitesinde 17,17±12,07, dermatoloji polikliniğinde 9,60±7,02 olup daha düşüktür. Depresif belirti görülme durumlarına göre aralarındaki fark istatistiksel olarak anlamlıdır. Katılımcılar yaşa göre tabakalandırılmıştır, 18-64 yaş arasında her iki gruptaki katılımcılarda depresif belirti görülme durumu arasında fark saptanmıştır(p<0,05). Diyaliz ünitesindeki 18-64 yaş grubundaki katılımcıların %36,8'inde, dermatoloji polikliniğindeki 18-64 yaş grubundaki katılımcıların %7,9'unda depresif belirti görülmüştür. Bu nedenle özellikle 65 yaşından genç yaştaki hastaların psikiyatrik durumları yakından takip edilmeli, gerekli durumlarda psikiyatrik danışmanlık, uzman desteği önerilmeli ve hastalar yönlendirilmelidir.
Abstract
Depression is one of the most common psychiatric disorders of hemodialysis treatment. The aim of this study was to compare the frequency of depressive symptoms of dermatology outpatients and hemodialysis patients. This cross-sectional study, hemodialysis patients with end stage renal disease and dermatology patients admitted to the outpatient clinics were included in the study. The data were collected with a questionnaire form and Beck Depression Inventory (BDI). Mean age of the participants was 50.09±17.02; 53.8% (n=57) were female, 67.0% (n=71) were married. Participants' mean age in the dialysis unit (n=36) was 62.06±14.03, participants' mean age in dermatology outpatients (n=70) was 43.94±15.10. BDI mean score of the participants in the dialysis unit was 17.17±12.07; BDI mean score of the participants in the dermatology outpatient clinic was 9.60±7.02. The difference between depressive symptoms of participants in the dialysis unit and in the dermatology outpatients' clinic was statistically significant (p<0,05). Participants were stratified according to age. Between the two groups frequency of depressive symptoms was statistically significant at 18-64 years. Depressive symptoms were seen in 36.8% of participants in the 18-64 age group in the dialysis unit, 7.9% of participants in the 18-64 age group in dermatology outpatients. For this reason, the psychiatric status of patients, younger than 65, should be monitored closely and if necessary psychiatric counseling or specialists support should be recommended and patients should be directed.
