Özet

Bu araştırma hemodiyaliz tedavisi gören hastaların hastalıkları ve hemodiyaliz tedavisi hakkındaki duyguları ve düşüncelerini belirlemek amacıyla gerçekleştirilmiştir. Kalitatif bir çalışma olarak yürütülmüştür. Özel bir diyaliz merkezinde hemodiyaliz tedavisi gören hastalardan çalışmaya katılan altı kişi (n=6) ile odak grup görüşmesi yapılmıştır. Veriler, hasta tanıtım formu ve yarı yapılandırılmış görüşme formu ile toplanmıştır. Görüşmeler odak grup görüşme tekniği ve ses kaydı kullanılarak kayıt altına alınmıştır. Katılımcıların deneyimleri, duyguları ve düşünceleri, kendi anlatımlarıyla ortaya konulmuştur. Veriler, içerik analiz yöntemiyle incelenmiş, kodlanmış ve temalar oluşturulmuştur. Katılımcılar, böbrek yetersizliği tanısını aldıklarında; şok, inkâr, kayıp, üzüntü yaşadıklarını, tedaviye ilk başladıklarında; korku, öz-yetersizlik, öfke, suçluluk duygusu hissettiklerini belirtmişlerdir. Çalışma yaşamlarının olumsuz etkilendiğini, dışlandıklarını ve yalnızlık yaşadıklarını bildirmişlerdir. Sonraki dönemlerde tedaviye uyum sağladıklarını ve hemodiyalizin onları yaşama bağladığını ifade etmişlerdir. Hastalar tarafından en sık yaşanılan sorunlar halsizlik, kramp, bulantı-kusma, susuzluk, kaşıntı ve hipo/hipertansiyon olarak bildirilmiştir. Bu sorunlar nedeniyle acı, ızdırap, kendini ifade etmede güçlük, üzüntü, huzursuzluk ve uykusuzluk yaşadıklarını ifade etmişlerdir. Hastalar, ruhsal sorunlara yönelik girişim bildirmemişlerdir. Fiziksel sorunlarda hemşirelerden destek aldıklarını belirtmişlerdir. Bu sonuçlar doğrultusunda hastaların hemşireler tarafından fiziksel, ruhsal ve sosyal yönden desteklenmesi oldukça önemlidir. Hemodiyaliz tedavisi gören hastalar için hastalık ve tedavi konusunda uyum programları düzenlenmesi önerilmektedir.

Abstract

This study was conducted to determine the emotions and thoughts of patients who were receiving hemodialysis treatment about their diseases and hemodialysis treatment. The research was a qualitative study. Focus group discussions were conducted with six individuals (n=6) among the patients who were receiving hemodialysis treatment at a private hemodialysis center. The data were collected using a patient introduction form and a semi-structured interview form. The interviews were conducted using the focus group discussion technique and recorded using a voice recorder. Experiences, emotions, and thoughts of the participants were presented with their own expressions. The data were analyzed using the content analysis method, coded and the themes were created. The participants stated that they experienced shock, denial, loss, and sadness when they were diagnosed with renal failure and that they felt fear, self-inadequacy, anger, and guilt when they first started treatment. They reported that their work life was negatively affected, that they were ostracized, and that they felt loneliness. They expressed that they complied to treatment subsequently and that hemodialysis let them hold on life. The most frequently experienced problems were fatigue, cramps, nausea-vomiting, thirst, itching, and hypo/hypertension. They expressed that they experienced pain, misery and difficulty in expression, sadness, discomfort, and insomnia due to these problems. Patients did not address any attempts related to mental issues. They reported that they received support from nurses for physical problems. In line with these results, it is very important to support patients physically, mentally and socially by nurses. For patients undergoing hemodialysis treatment, compliance programs for disease and treatment are recommended.
