Özet
Amaç: Gözlenen epidemik influenza enfeksiyonu sonrası üç vakada postviral optik nöropatinin incelenmesi. Materyal ve Metod: influenza benzeri hastalıktan birkaç hafta sonra, kliniğimize aynı hafta içinde başvuran 3 hastada klinik olarak postenfeksiyöz optik nöropati tanısı konulmuştur. İki hastada görme keskinliği çok az etkilenmiş, fakat görme alanı defektleri gelişmiştir. Bulgular: Hastaların birinde postkontrast yağ baskılamak magnetic rezonansda (MR) optik sinir demyelinizasyonu gösterilebilmiştir. Bu hastalar tedavisiz iyileşmişlerdir. Bir hastada ise, her iki gözde ilerleyici görme kaybı gelişmesi üzerine acil medikal tedavi uygulanmış ve bu hastada da tam iyileşme olmuştur. Sonuç: Bölgemizde influenza epidemisi sonrasında, tipik öykü ve klinikle aynı hafta içinde başvuran bu hastalar; bu tip optik nöropatilerin tedavi endikasyonunu tartışmak üzere sunulmuştur.
Abstract
Purpose: Evaluation of the post-viral optic neuropathy seen in three patients during an epidemy of influenza infection. Methods : Three patients were seen with loss of vision and visual field defects following an influenza-like disease. Two patients had moderate loss of vision and visual field defects. Results : Demyelinization of the optic nerve on fat suppression post-contrast MRI was de-monstrated in one of these patients. These two patients recovered spontaneously. A third patient; wo had bilateral optic neuritis had complete recovery in a week following IV methylprednisolone treatment. Conclusion: The presenting symptoms and signs of these patients are of postviral optic neuropathy. These patients are presented as examples of this type of optic neuropathy and treatment strategies in similar cases are discussed.
