Özet

Amaç: Fakoemülsifikasyon cerrahisi bağlı vitreusa lükse lenslerin pars plana vitrektomi (PPV) ile çıkarılması ve aynı seansta iris kıskaçlı göz içi lens (GİL) implantasyonu sonuçlarını değerlendirmek. Gereç ve Yöntem: Çalışmaya aynı seansta PPV ile birlikte iris kıskaçlı GİL implantasyonu uygulanan 10 hastanın 10 gözü dâhil edildi. Olguların düzeltilmemiş ve düzeltilmiş en iyi görme keskinlikleri, biyomikroskopik ve fundoskopik bulguları, göz içi basınç (GİB) değerleri ve cerrahi komplikasyonlar geriye dönük olarak incelendi. Bulgular: Hastaların 6’sı erkek, 4’ü kadındı. Hastaların ortalama yaşı 65.10±11.68 (54-87) yıldı. Hastaların ortalama takip süresi 8.10±2.7 (6-12) aydı. Ameliyat sonrası son kontrol muayenesinde 9 (%90) hastada görme keskinliğinde Snellen eşeline göre en az 3 sıra artış saptandı. Ameliyat sonrası dönemde kistoid makula ödemi (n=1), geçici GİB artışı (n=2) ve pupillada oval görünüm (n=1) izlendi. Tartışma: Vitreusa lens lüksasyonlarında PPV ile birlikte aynı seansta iris kıskaçlı GİL implantasyonu afakinin tedavisinde güvenle uygulanabilmektedir. Ancak afakinin tedavisinde kullanılan bu tür lenslerin etkilerinin değerlendirilmesi için daha geniş serili, prospektif ve randomize çalışmalara ihtiyaç duyulmaktadır.

Abstract

Purpose: The aim of this study was to evaluate the results of iris-claw intraocular lens (IOL) implantation at the same session combined with pars plana vitrectomy (PPV) for the extraction of luxated lenses into the vitreous due to phacoemulsification surgery. Materials and Methods: 10 eyes of 10 patients that underwent pars plana vitrectomy and iris-claw IOL implantation at the same session were included in this retrospective study. Uncorrected and best corrected visual acuities, biomicroscopic and fundoscopic findings, intraocular pressure (IOP) measurements, and surgical complications were evaluated retrospectively. Results: Six patients were male and four patients were female. The mean age was 65.10±11.68 (54-87) years. The average time of follow up was 8.10±2.7 (6-12) months. At the final examination 9 (90%) eyes showed at least three snellen lines improvement in the visual acuity. At the postoperative period, cystoid macular edema (n=1), transient IOP increase (n=2), and pupil ovalization (n=1) occured. Conclusion: Iris-claw IOL implantation combined with PPV at the same session for the luxated lenses into the vitreous can be used safely for the treatment of aphakia. However, large series of prospective randomized studies are needed to clarify this type of IOLs for the their effectiveness for the treatment of aphakia.
