Komplike maküla tutulumlu regmatojen retina dekolmanında pars plana vitrektomi sonrası koroid kalınlığı

Özet
Amaç: Pars plana vitrektomi (PPV) uygulanan komplike maküla tutulumlu regmatojen retina dekolmanı (RRD) olguların- da vitrektomi esnasında intravitreal silikon veya C3F8 enjekte edilen olgularda spektral domain-enhanced depth imaging- Optik koherens tomografi (SD- EDI-OKT) ile postoperatif subfoveal koroid kalınlığını (SKK) incelemek. Gereç ve Yöntem: Komplike maküla tutulumlu RRD tanısı konarak, PPV uygulanan 18 göz retrospektif incelendi. SKK, SD-EDI-OKT ile 2 grupta (Grup 1: PPV+Laserfotokoagülasyon+C3F8; n=10 göz; Grup 2: PPV+Laserfotokoagülasyon+Silik on; n=8 göz) postoperatif 1. hafta, 1. ve 6. ayda incelendi. Gruplar kendi aralarında ve opere gözler cerrahi işlem görmemiş sağlıklı göz ile karşılaştırıldı. Bulgular: SKK Grup 1 ve 2de postoperatif 1. hafta, postoperatif 1. ve 6. aya göre istatistiksel anlamlı daha kalın ölçüldü. SKK, PPV uygulanan gözlerde postoperatif 1. ve 6. ayda cerrahi işlem görmemiş sağlıklı gözlere göre farklılık göstermedi. Postoperatif 1. hafta, 1. ve 6. ayda gruplar arasında SKKnda fark görülmedi. Sonuç: Komplike maküla tutulumlu RRD olgularında postoperatif 1. hafta PPV+Laserfotokoagülasyon+C3F8 ve PPV+La serfotokoagülasyon+Silikon uygulanan gözlerde SKKnda artış görüldü. Erken postoperatif dönemde görülen SKK artışı laserfotokoagülasyonun geçici skleral koroidal inflamatuvar etkisinden kaynaklandığı düşünülmektedir. Postoperatif geç dönemde SKK ında gruplar arasında fark olmaması silikon ve C3F8in farklı etki yaratmadığını düşündürmektedir.
Abstract
Purpose: To investigate subfoveal choroidal thickness (SCT) between pars-plana vitrectomy (PPV)+laserphotocpagulation +C3F8 and PPV+ laserphotocoagulation+silicon oil for complicated macular-off rhegmatogenous retinal detachment (RRD) postoperatively using enhanced depth imaging (EDI) spectral domain optical coherence tomography (SD-EDI-OCT). Materials and Methods: The medical charts of 18 patients (18 eyes) who underwent PPV+laserphotocoagulation+C3F8 (Group-1; n=10 eyes) versus PPV+laserphotocoagulation+silicon oil (Group-2; n=8 eyes) for complicated macula-off RRD were retrospectively analyzed. Postoperative SD-EDI-OCT images concerning SCT were evaluated 1 week, 1 month and 6 months postoperatively in 2 groups. SCT was compared between the groups and with the non-operated fellow eye. Results: SCT in groups 1 and -2 was thicker 1 week postoperatively compared with SCT after 1- and 6 months. SCT after PPV was not different 1- and 6 months postoperatively when comparing the operated eye with the non-operated fellow eye. There were no significant differences between the groups 1 week, 1- and 6 months postoperatively. Conclusion: SCT after PPV+laserphotocoagulation+C3F8 and PPV+ laserphotocoagulation+silicon oil in cases with com- plicated macular-off RRD was thicker 1 week postoperatively. Thicker SCT 1 week postoperatively may most likely by temporary scleral and choroidal inflammation after laserphotocoagulation during PPV. Postoperatively after longer period there was no significant difference in SCT in two groups, that we conclude silicon oil or C3F8 did not influence SCT different.
