Özet

Vogt-Koyanagi-Harada sendromu (VKH) nedeniyle takip edilen 4 hastanın klinik özelliklerini ve tedavi sonuçlarını değerlendirme- yi amaçladık. Dört kadın hastanın yaşları 33-58 yaş, takip süre- leri 8-49 ay arasında değişmekteydi. Olguların tümünde bilateral tutulum mevcuttu. İşitsel şikâyetler üç olguda (1 olguda işitme azlığı, 2 olguda çınlama) mevcuttu. Olguların tümü spektral do- main optik koherens tomografi, fundus fl oresein anjiyografisi ve ultrasonografi ile değerlendirildi. Üç olgu “tam olmayan” ve bir olgu da “muhtemel” VKH olarak sınıfl andırıldı. Tüm olgularda bilateral ön üveit ve eksüdatif retina dekolmanı mevcuttu. Op- tik disk ödemi 3 olguda bulunmaktaydı. Tüm olgular yüksek doz kortikosteroid tedavisi ile tedavi edildi. İlaveten iki olguya siklosporin, birine azatioprin verildi. Takiplerde 1 olguda iki kez nüks izlendi, 1 olguda koroidal neovasküler membran, 1 olguda pupiller blok, neovasküler glokom ve katarakt ve 1 olguda sekon- der glokom gelişti. Vogt-Koyanagi-Harada sendromlu olgularda erken tanı ve tedavi ile görme kaybı engellenebilir. Sendromun tedavisinde kortikosteroidler etkili olup; kronik, kortikosteroid tedavisine dirençli ve şiddetli kortikosteroid yan etkileri gelişen olgularda immunsupresif ilaçlar faydalı olabilmektedir. Olası komplikasyonlar ve nüksler açısından VKH’lu hastalar yakın ta- kip edilmelidir.

Abstract

We aimed to evaluate the clinical features and treatment results in 4 cases with Vogt-Koyanagi-Harada syndrome (VKH). Four fe- male cases’age range was 33-58 years, follow-up duration 8 to 49 months. All cases had bilateral involvement. Auditory compla- ints were available in three cases (hearing loss in 1 case, tinnitus in 2 cases). All cases were assessed by spectral domain optical coherence tomography, fl uorescein angiography and ultrasonog- raphy. Three patients were classified as "incomplete VKH" and one as "probable VKH”. Baseline visual acuity ranged from 2/20 to 3/20 in the RE and from 2/20 to 4/20 in the LE. All cases had bilateral anterior uveitis and exudative retinal detachment. Op- tic disc edema was also present in three cases. All patients were treated with high-dose corticosteroid therapy. In addition, cyc- losporine was administered to two cases and azathioprine to one case. Final visual acuity ranged from light perception to 20/20 in the RE and from 12/20 to 20/20 in the LE. During the follow-up, recurrence was observed twice in 1 case; choroidal neovascular membrane in 1 case, pupillary block, neovascular glaucoma and cataract in 1 case and secondary glaucoma in 1 case. Visual loss could be prevented by early diagnosis and treatment in VKH pa- tients. Corticosteroid is eff ective in treatment of VKH. Immuno- suppressive drugs could be useful in patients with a chronic VKH refractory to corticosteroid or who suff er severe adverse eff ects with corticosteroid. VKH cases should be followed closely with regard to possible complications and recurrence.
