Yırtıklı Alt Yarı Retina Dekolmanında Vitreon Tamponat Kullanımı: Cerrahi Sonuçlar ve Komplikasyonlar

Özet
Amaç: Alt yarı yırtıklı retina dekolmanı (YRD) nedeniyle vitreoretinal cerrahi (VRC) uygulanan ve tamponat olarak vitreon tercih edilen olguların postoperatif anatomik ve fonksiyonel başarı durumları, postoperatif dönemde görülen komplikasyonları ve bunların sonuçlarını incelemek. Gereç ve Yöntem: Kırk üç hastanın 43 gözü retrospektif olarak değerlendirildi. Hastaların retina dekolmanı etyolojisi, ameliyat öncesi oküler hastalıklar, düzeltilmiş en iyi görme keskinliği, göz içi basıncı (GİB) değerleri, biyomikroskopik özellikleri, detaylı fundus muaye- nesi bulguları, postoperatif dönemde görülen komplikasyonlar, anatomik ve fonksiyonel başarı durumları değerlendirildi. Bulgular: Çalışmaya alınan 43 olgunun 26’sı erkek (% 62,8), 17’si kadın (% 37,2) olup olguların ortalama yaşı 50,8±18,8 idi. On (%23,3) hastada YRD için herhangi bir predispozan faktör bulunmazken, tespit edilebilen en sık sebep psödofaki idi (13 hasta-%30,2). Postope- ratif dönemde 20 hastada görme artışı sağlanırken, 13 hastada ilk görme keskinliği durumu korundu. 10 hastada ise preoperatif görme keskinliğinden daha kötü bir görme düzeyi elde edildi. Vitreon çıkarılması sonrası % 39,5 oranında nüks görüldü. Postoperatif anatomik başarı oranı % 60,5 iken, fonksiyonel başarı oranı % 76,7 idi. Postoperatif dönemde görülen en sık görülen komplikasyon sekonder kata- rakttı (%23,3). Sonuç: Vitreon, postoperatif dönemde çeşitli komplikasyonlara yol açması ve nüks oranlarının fazlalığından dolayı, alt yarı yırtıklı dekol- man olgularında intravitreal tamponad olarak kullanımı öncelikli olarak düşünülmemelidir.
Abstract
Purpose: To evaluate the postoperative anatomical and functional success status, postoperative complications and their results of vitreore- tinal surgery (VRC) with the preference of vitreon use as a vitreoretinal (VR) tamponade for inferior rhegmatogenous retinal detachment (RRD). Methods: Forty-three eyes of 43 patients were retrospectively evaluated. Patients’ etiology of retinal detachment, preoperative ocular diseases, best corrected visual acuity (BCVA), intraocular pressure (IOP) values, biomicroscopic features, detailed fundus examination fi ndings, postoperative complications, anatomical and functional success results were evaluated. Results: Twenty six of the 43 cases were male (62,8%) and 17 were female (37,2%). The mean age of the cases was 50.8±18.8. Ten (23.3%) patients had no predisposing factor for RRD, whereas the most common cause was pseudophakia (13 patients - 30.2%). In the postoperative period, visual acuity (VA) was increased in 20 patients and the initial VA was preserved in 13 patients. In 10 patients, visual acuity was worse than preoperative VA. After removal of the vitreon, the recurrence rate was %39.5. Postoperative anatomical success rate was 60,5%, and functional success rate was 76,7%. The most common postoperative complication was secondary cataract (23,3%). Conclusion: Vitreon should not be considered primary as an intravitreal tamponade in the case of for inferior rhegmatogenous retinal de- tachment due to various complications in the postoperative period and the high recurrence rates.
