Familyal Eksüdatif Vitroretinopatili Hastaların Tanı, Takip Ve Tedavisinde Geniş Açılı Fluoresein Anjiografinin Yeri

Özet
Amaç: Kliniğimize refere edilen familiyal eksüdatif vitroretinopatili (FEVR) klinik tanılı hastalarda, flöresein anjiyografi bulgularının hastalığın tanı, sınıflandırma, takip ve tedavisindeki katkılarının incelenmesi ve sonuçların bildirilmesi amaçlanmıştır. Materyal metod: Klinik bulgularına dayanılarak FEVR tanısı almış 14 hastanın 28 gözü çalışmaya dahil edildi. Tüm hastalara sedasyon altında geniş açılı flöresein anjiyografi (FA) tetkikleri RetCam III (Imaging System Clarity Medical Systems, Pleasanton, CA, USA) kullanılarak yapıldı ve evreleme anjiografi bulgularına dayanılarak gerçekleştirildi. Tüm bulgular retrospektif olarak incelendi. Bulgular: Çalışmaya dahil edilen hastaların doğum haftaları ortalama 38,57±1,45 (36-40) hafta, doğum kiloları ortalama 2745±731,09 (800-4000) gram ve ortalama yaşları 47,75 ±51,09 (3-168) ay olarak kaydedildi. Tüm gözlerin 11’inde (%39,28) evre 1a, 2’sinde (%7,14) evre 1b, 1’inde (%3,75) evre 2a, 11’inde (%39,28) evre 2b, 1’inde (%3,75) evre 3, 2’sinde (%7,14) evre 5 FEVR tespit edildi. Flöresein anjiyografi de neovaskülarizasyona bağlı sızdırma tespit edilen tüm gözlere lazer fotokoagulasyon (LFK) uygulandı. İki hastanın birer gözü evre 5 FEVR sebebiyle vitroretinal cerrahi geçirmişti. Bu hastaların diğer gözlerine tespit edilen evre3 ve 1a hastalık tanıları ile LFK uygulandı. Sonuç: Geniş açılı flöresein anjiyografi ; familiyal eksüdatif vitroretinopati hastalığının tanısının konulması, tedavi kararı ve takip sürecinde önemli bir yere sahiptir. Özellikle 5 yaş altı çocuklarda sedasyon altında uygulanması sayesinde, FEVR hastalığının görmeyi tehdit eden komplikasyonları henüz gelişmeden progresyon riskinin tespit edilmesine olanak sağladığından kıymetlidir.
Abstract
Purpose: The aim of this study was to investigate the contribution of fl uorescein angiography fi ndings to the diagnosis, classifi cation, follow-up and treatment of the disease in patients with the diagnosis of familial exudative vitreoretinopathy (FEVR) that were referred to our clinic. Materials and Methods: Twenty-eight eyes of 14 patients diagnosed with FEVR based on clinical findings were included in the study. Wideangle fl uorescein angiography (FA) was performed with the RetCam III (Imaging System Clarity Medical Systems, Pleasanton, CA, USA) in all patients under sedation and staging of the disease was performed based on angiographic findings. Results: The mean gestational age of the patients in the study was 38.57 ± 1.45 (36-40) weeks, the mean birth weight was 2745 ± 731.09 (800-4000) grams and the mean age was 47.75 ± 51.09 (3- 168) months. Eleven eyes (39.28%) had stage 1a, 2 eyes (7.14%) had stage 1b, 1 eye (3.75%) had stage 2a, 11 eyes (39.28%) had stage 2b, 1 eye (3.75%) had stage 3 FEVR and stage 5 FEVR was detected in 2 eyes (7.14%) of 2 patients. Laser photocoagulation was performed to all eyes with leakage on fl uorescein angiography. Two eyes of 2 patients had vitreoretinal surgery for stage 5 FEVR. Laser photocoagulation was applied to the fellow eyes of these patients with the diagnoses of stage3 and 1a disease. Conclusion: Wide-angle fl uorescein angiography is important in the diagnosis, treatment decision and follow-up of FEVR. With sedation for children under 5 years of age, the detection of the risk of progression before development of vision-threatening complications of FEVR is possible.
