Hastanede Çalışan Sağlık Personeline Hasta/Yakınları Tarafından Uygulanan Şiddet: Nedenleri ve İlgili Faktörler

Özet
GİRİŞ: Son yıllarda hastanelerde sağlık personeline uygulanan şiddet olayları artmıştır.AMAÇ: Hastanelerde çalışan sağlık personeline, hasta/yakınları tarafından uygulanan şiddet durumunu, şiddetin nedenlerini ve ilgili faktörleri belirlemektir. YÖNTEM: Araştırma, Trabzon'da Sağlık Bakanlığına bağlı 12 hastaneye bir gün içinde başvuran 770 hasta ve bu kurumlarda çalışan 798 sağlık personeli ile yürütülmüştür. Veriler, hastalar ve sağlık personeli için ayrı geliştirilen, her iki grubunda demografik özellikleri ve sağlık personeline uygulanan şiddet ve nedenlerine yönelik soruları kapsayan anket ile toplanmıştır. BULGULAR: Hastaların %35'i, sağlık personelinin %74'ü hastanelerde sağlık personeline şiddet uygulandığını görmüş ve her iki grup da şiddetin sözlü gerçekleştiğini açıklamıştır. Polikliniğe başvuran ve üniversite mezunu hasta/yakınları şiddeti daha çok hekimlerin ve tıbbi sekreterlerin hak ettiğini düşünürken (p<0,05), acile başvuran hasta/ yakınları şiddeti daha çok hemşirelerin hak ettiğini düşünmektedir (p<0,001). Ayrıca hastaların %52'si ve sağlık personelinin %68'i uygulanan şiddetin sağlık sisteminden, hastaların %36'sı ve sağlık personelinin %58'i hasta/yakınlarından, hastaların %30'u ve sağlık personelinin %15'i sağlık personelinden kaynaklandığını belirtmiştir. Her iki grupta hasta/yakınları tarafından uygulanan şiddetin sistem kaynaklı nedenini; öncelikle poliklinik/muayene için bekleme sürelerinin uzamasına, hasta/ yakınları kaynaklı nedenini; hasta/yakınlarının sabırsız ve anlayışsız davranmasına ve sağlık personeli kaynaklı nedenini; sağlık personelinin ilgisiz/ duyarsız olmasına ya da hastaların ciddiye almamasına bağlamıştır. SONUÇ: Hastanelerde hasta/yakınları tarafından sağlık personeline başta sözlü olmak üzere şiddet uygulanmakta ve şiddet olayları daha çok sistem yetersizliğinden kaynaklanmaktadır.
Abstract
INTRODUCTION: Violence committed against health personnel in hospitals has increased in recent years.AIM: To determine the reasons and factors regarding violence towards health personnel working at hospitals commited by patients/their families.METHOD: The study was conducted with 770 patients consulting 12 hospitals of the Ministry of Health in Trabzon in a day's time as well as 798 health personnel working at these institutions. The data were collected between March 7th and 28th, 2012 by questionnaires developed separately for the patients and health personnel which composed of questions regarding the demographic characteristics of both groups and the violence committed against the health personnel and its reasons. RESULTS: A 35% of the patients and 74% of the health personnel stated that they had witnessed violence against health personnel at hospitals and both groups clarified that the violence they had witnessed was mostly verbal. University graduate patients/their relatives who consulted an outpatient unit believed that mostly doctors and medical secretaries had deserved violence (p<0.05) whereas patients/their relatives consulting an emergency room thought that mostly nurses deserved violence (p<0.001). In addition, 52% of the patients and 68% of the health personnel noted that the violence had originated from the health system. On the other hand, 36% of the patients and 58% of the health personnel stated that it had stemmed from the patients/their relatives while 30% of the patients and 15% of the health personnel indicated that the health personnel was the reason for violence. In both groups, system-induced violence was primarily correlated with the prolonged waiting time for examination. Violence inflicted by patients/their relatives was correlated with the impatient and unsympathetic behaviour of patients/their relatives; and violence committed by health personnel was associated with the insensitive and indifferent behaviour of the health staff or their lack of attention to the patients.CONCLUSION: It has been determined that violence, which is mostly verbal, is committed against the health personnel by patients/their relatives and most of these events occur due to the insufficiencies within the health system.
