Özet
GİRİŞ: Sağlık sistemleri, ekonomik sistemlerden doğrudan etkilenen birer alt sistem olup ülkelerin ekonomik sistemleri değiştikçe sağlık sistemleri de değişebilmektedir. Ekonomik sistemin ne olduğuna, bu sistemin sağlık sistemine nasıl etki ettiğine bakılmaksızın, sağlık sistemlerinde çözülmesi gereken temel sorunun başında sağlığa erişim, adalet, hakkaniyet, kalite gibi konular gelmektedir. Sistemler, bu tür sorunların çözümüne uzak kaldıkça sistemde yer alan bireyler kendi yöntemlerini sistemdeki açıkları gidermek amacıyla uygulamaya başlamaktadır. Bu anlamda informal ödemeler, hem sağlık çalışanları hem de sağlık hizmeti tüketicileri için bu tür zayıflıkların kullanılması açısından birer araç olarak görülebilmektedir. AMAÇ: Bu çalışmanın amacı, sağlık sistemlerinde erişim ve kaliteli hizmet alma noktasında çeşitli adaletsizliklere ve sistemin yozlaşmasına neden olabilen informal ödemelere ilişkin bir çerçeve çizilmesidir. YÖNTEM: Çalışmada yöntem olarak, sistematik literatür taraması yöntemi yeğlenmiş, derlemeye kabul edilme ölçütlerini sağlayan toplamda 20 çalışma incelenmiştir. BULGULAR: Çalışmada, informal ödemelerin özellikle post-komünist ülkelerde ve gelişmişlik düzeyi düşük olan ülkelerde yüksek bir yaygınlık oranına sahip olduğu belirlenmiş ve bunun nedenleri irdelenmiştir. SONUÇ: İnformal ödemelerin kayıt altına alınabilmesi için birtakım çalışmaların gerçekleştirilmesi ve Türkiye’de atılan adımların sonuçlarının izlenebilmesi için informal ödemelerle ilgili derinlemesine çalışmaların yapılması önerilebilir.
Abstract
INTRODUCTION: Health systems are subsystems directly affected by economic systems. Therefore, health systems may change as the economic systems of countries change. Regardless of which economic system is adopted and how this system is effecting the health system, problems such as access to health, justice, equity and quality are the main problems that should be solved in health systems. As systems fail to solve such problems, individuals within the system begin to implement their own methods with the aim of resolving deficits in the system. In this sense, informal payments can be seen as a tool for the compensation of these deficits by both health workers and healthcare consumers. AIM: The aim of this study is to draw a frame for informal payments which may cause various injustices and degeneration of system in terms of access to health systems and quality of health services. METHODS: Systematic literature review was preferred as a method in the study and 20 studies which met inclusion criteria were included in the study. RESULTS: Informal payments have a high prevalence especially in post-communist and underdeveloped countries and the reasons of these results were examined. CONCLUSION: It is recommended that a number of studies should be carried out to legitimize these kind of payments, and to monitor the results of the precautions taken into consideration in Turkey.
